Hi all, I am not really new here but new to posting. Tonight or maybe in the morning I am going to switch my kitty from Lantus to Levemir. Shelby is on 1.25 units of Lantus. My vet suggested doing 1/3 of her previous dosage when I start the Lev. I just made a new spreadsheet just for the Lev, I don't have a spreadsheet for her three years on Lantus due to multiple issues but I always kept a diary of her numbers. I shared a more in depth history in one of the other forums.

So for now I am just asking what to start her on. I am good with testing, and even shooting low numbers. I know Lev has a later nadir than Lantus but Shelby has always tended to nadir more between +6 and +8 on the Lantus. I do work tomorrow night, so I'll be gone about 6 hours. I usually shoot her between 1030/11 AM/PM depending on mine and the kids schedules.

When switching from Lantus to Lev, we generally recommend starting Lev at 3/4 of the Lantus dose. If you want to reduce further, that's fine. Some cats do have a marked response to a switch in insulin. Remember, though, it will take a week for the Lev depot to get established.

Keep in mind that Levemir typically has a later nadir than Lantus as you noted. Generally, cat's will have a nadir at around +8 and sometimes later in the cycle. If you don't already have one, you might want to get a timed feeder since it could be quite late when Shelby is reaching nadir.

Thank you. I gave her her last Lantus shot this morning. She is due in an hour, do I switch directly to the Lev or do I let her Lantus depot run out?
