? Switching cat from Prozinc to Basaglar pen

Hi Everyone,

Non-urgent post here. I have a sugar baby who has been on Prozinc since 2018. I think she needs a longer lasting insulin because her glucose curve is a definitie U (coming up to the high 300s at the end of a cycle, but with nice nadirs 6 hours in). I test her glucose levels every day. She's doing really well, otherwise. She had one DKA in 2020 possibly due to a combination of a UTI, stress from a long drive, and care-taker possibly doing insulin shots incorrectly. Nothing since then, thankfully. She eats fancy feast pate and occaisionally gravy to entice her if she doesn't want to eat.

I've read all the posts and the guidance on here (so helpful!) and I just want to confirm that my plan to move her onto Basagler from Prozinc sounds okay:

1. I'm going to order the Basaglar Kwikpen (single) from Mark's Marine in Canada (with the cooler and extra ice packs because I'm in Florida. I'm considering doing their Rapid Airmail too), just to see how she does on it before putting in a larger order. It doesn't look like I need a prescription for this, which is great.

2. I'm going to purchase U-100 needles, probably from Amazon. Currently my baby gets between 1.75 and 2 units of Prozinc on the U40 needles. She will get between 1.75 and 2 units of Basaglar using the U100 needles. I will not put insulin from the syringe back into the pen in case of contamination. The pen is guaranteed for 28 days, but will probably last quite a bit longer if I'm careful and keep it refrigerated (I always refrigerate insulin anyway).

3. Her blood sugar may be a bit high initally as the depot builds up. The shots need to be every 12 hours on the dot (no flexibility as with Prozinc). I will test every two-three hours during the first couple of days to monitor how she is responding, and I will also check her ketones regularly.

Does this sound right? Is there anything I'm missing? Thanks in advance -- this group has literally saved two lives, hers and mine. I'm so grateful for you all.

That 28 day is for humans , one pen should last you at least 6 months kept in the middle shelf of the fridge

I would also suggest to get some tests after the PMPS test, cats usually drop lower at night
Only testing during the day you are only seeing half the picture :cat:
Lantus lasts much longer than 28 days! That's only for humans who doesn't store it in the fridge.
Just use the pen up.

If you have a Walmart by you they also have U-100 syringes with half unit markings
You can also get the syringes with half unit markings at a Walmart. Their ReliOn Brand syringes of 0.3 cc (30 unit) capacity come with half unit markings. Cost $12.58 for a box of 100

They won't say it half unit on the box but the are . open then and look before you leave the store, can't be ordered on line , need to go into the store

Hi Belinda , as far as I know you don't need a prescription if you live in Canada to purchase insulin
In the US I'm certain you need a script from your vet, Marks will contact them and they can fax the script over to them, after that you don't need a script to order it again

Or you can get generic lantus from a pharmacy
You can have the vet write the script for generic lantus ,many members use it, The brand is much more expensive, just call around and see who might have the generic

The 5 pens will last about a year, we use the pens just like a vial, you would just insert the syringe it the gray rubber stopper on the pen and draw out your insulin

Any chance you can put the last couple weeks of Prozinc data in your spreadsheet? (your meter should keep data that long). With that data, we'll be able to help suggest a starting glargine (Basaglar) dose.

There is a little bit of wiggle room with Lantus (and the biosimilars). You can move the shot time 15 minutes each shot, or 1/2 hour once per day.

Belinda I'm confused , I looked at your SS tab for 2022 I tapped on it and what comes up are dates that are listed starting with 1-1-2023 and with 7-20-2023?
We haven't even gotten to 7-20-2023 yet
Did you mean 2022 for that tab up top?

Then on your tab up top for 2023 starting with 1-1-2023 you have no Data listed
Up top of the spreadsheet you have
Insulin: Prozinc switching to Basaglar when do you plan on switching ?
Do you have any tests for 1-1-2023 for using Prozinc still?
