Switch from Prozinc to Lantus

Pudge saw a new vet today who recommended switching from a fatter 1.75u dose of ProZinc to 1u Lantus. We will be starting him on Lantus tomorrow. I have read the stickies on this forum for Start Low and Go Slow and Tight Regulation. Is there any major differences I should be aware of that the vet may have failed to mention? I’m nervous to do something wrong and I feel comfortable with ProZinc so Lantus is very new to me so I don’t want to mess anything up!

TR gives the best chance at remission, but it requires more testing. Reductions are earned when they drop below 68 on the Alpha Trak.

Besides PS tests, you need to get at least 1 mid-cycle on the AM cycle (if possible) and at least a "before bed" test on the PM cycle.

Doses can be changed as often as every 3 days if they aren't getting them where we want them.

On SLGS, reductions are earned when they drop below 90 and doses are held for 7 days. You still always have to do PS tests and at least a "before bed" test (since most cats go lower at night) but before you change the dose, you should run at least 1 curve.....testing every 2 hours for 12 hours or every 3 hours for 18 hours.

You have to hold the starting dose of Lantus for 5-7 days before you'll know how well they're really going to do....although occasionally we'll see a cat that reacts sooner.
 
Hello and welcome. Typically we consider the dose of the current insulin when switching to Lantus. 2.0 units got him to low green, so maybe 1.5 would be a better starting point to give you some safety. Make sure you can monitor closely the first couple cycles, in case Pudge is one of those cats that reacts strongly to a change in insulin.
 
The other difference between Lantus and Prozinc is that Lantus dosing is based on the lowest point in the cycle (nadir) and not the pre-shot test. Pre-shot information is essential so you know whether it's safe to give insulin but you do not slide the dose based on the pre-shot number. Doses are reduced if they fall below 50 with TR or below 90 with SLGS and doses are held for several days in order for the insulin depot to stabilize. (Lantus is a depot type of insulin; Prozinc is not.)

Please make sure you have U100 syringes. Prozinc syringes are U40.
 
Thank you all so much! I felt like I was missing some information so I am glad I asked. I am going to pick up u100 needles today and then I will start posting some numbers in the next few days

We used ProZinc for 6 months and switched to Lantus and was and is very happy I did. Lantus has an actual protocol which helped me know what to do where with ProZinc, I was always guessing.

As others have said, check out the protocol Tight Regulation( TR) and the method of Start Low Go Slow ( SLGS) It appears that you test enough to do TR and as Chris said, you can get a cat regulated and or into remission using TR.

Please post daily with a new thread with your cat's name, date and AMPS and peeps here will help you. And ask questions as that is how we all learned.

That’s my biggest concern with Prozinc is I constantly feel like I am guessing. I am glad to hear others have made the switch and are happy with it. I am nervous but hoping it will be better for Pudge. Thank you :)

Obviously every cat is different, but I switched from Vetsulin to Basaglar (glargine insulin similar to Lantus), and I am beyond pleased with my buddy Lou's progress. Although I will be switching Lou to Levemir this month (he's a bouncy rainbow kitty), I have not had a single regret switching to a depot insulin. Just know there is a learning curve. I felt like I was brand spankin new again, haha. But the stickies are great for learning and reviewing the process, and the people on this form are fantastic and more than willing to share their knowledge. You got this!

That’s exactly how I feel! I felt like I was finally starting to understand the ins and outs of prozinc and Lantus is a whole new world. Thank you for your encouragement!
