Well.. I called the vet today with numbers from the weekend. We started Lantus insulin this evening 3units every 12hrs. Mr. F. sugars were as follows today.

5am it was 533.

523pm it was 423, gave 3 units of lantus.

755pm (+2hrs) its 457.

Do i need to give him more insulin? Do i only give insulin every 12hrs, even if the #s are high?

I understand it's not going to happen right away, just want to make sure he's getting what he needs.

How long has it been since Focker has been on the all low carb wet diet? 3 Units twice a day is a pretty high starting dose and it's going to be important to get mid-cycle test to see how low that dose is taking his blood sugar. Lantus is injected every 12 hours.

One thing you'll need to try to understand about Lantus....it's a "depot" insulin...when it's first started, it usually takes 5-7 days before you're going to see the full effect of the dose you're giving.

Here's the "technical" jargon:
After you inject it into your subcutaneous tissue, the acidic solution is neutralized by your body to a neutral pH. Because glargine is not soluble at a neutral pH, it precipitates out into a form that's not soluble in subcutaneous fat, and there forms a relatively insoluble depot. From that pool, or depot, of precipitated glargine in the tissues, small amounts slowly move back into solution over time and then to the bloodstream.

In simpler terms, when you first start, only a little goes "to work"....the rest goes into the "depot" and slowly releases into the bloodstream. But until the "depot" is completely full, it can't "release" an amount in relationship to the dose...so it will take several days before the depot will release the entire dose over the 12 hour cycle.

3U is a really high starting dose. Lantus has a formula for the starting dose that's Weight (in kgs) x .25....so unless Mr. F weighs about 30lbs, 3U is way too much. If you choose to continue giving that much, please keep a close eye on him and test as much as possible. Make sure you also have a "hypo kit" ready in case you need it.

We've seen lots of cats here that were started out at too high a dose and they ended up in an ER (or worse) due to severe hypoglycemia.

High numbers make us worry, but they take time to do damage....it's the low numbers that kill quickly.

You only give insulin every 12 hours.
NEVER give more insulin inbetween the 12 hour cycles.

Have you swapped from the dry food over to the low carb wet food now?

3 units is much too high a dose for Mr F to start on. How much does he weigh?
Unless he is a very heavy cat, I would reduce the dose back down to 1 unit - 1.5 units, depending on his weight, and stay with that dose for 5 days unless he drops below 50 in which case you will need to reduce the dose.

Please make sure you test frequently during this current cycle to ensure Mr F doesn't drop too low.
I know this will mean you will have to set the alarm to check his blood sugar levels but I think it is important considering the 3 unit dose. I would check at +4 and +6 to start with

The increase from 423 to 457 at +2 was probably a food bump.
 
At 506am this morning it was 180, i hope giving 1u was ok....

Its hard to check after post dose, cause my husband and i both work 6am-430p, so were out the door by 530a... i feed him and dose him between 5a-530a, so that when we get home its time to do another dose and eat.

Mr. F has been on wet food since Saturday afternoon, no dry food since.
 
Did you leave some food out for him to snack on during the day?

Are you able to get home at all during the day to check on him? At lunchtime maybe?
 
Good! Try to get at least 1 more test in on the PM cycle....if you can get more, all the better!

It's nice to see some blue and yellow instead of those reds and blacks!
