Spreadsheet review-up dose?

Hello,

I need some input on Squeak's numbers. I switched over to Levemir 7 days ago and as you can see, I'm seeing a lot more yellow numbers than we did on Prozinc. My hunch is my 'drop doses' on the u40 syringe were more like 'fat doses' and he's actually getting less insulin now that I have a more accurate way to get a drop dose, therefore, the higher numbers. What should I do?? TYIA for your help!

There really shouldn't be a difference in the dosing since the concentration of the insulins match the syringe (i.e., you use a U40 syringe with Prozinc because it's a U40 insulin and a U100 syringe with Lev because it's a U100 insulin). What I suspect is that you've been holding the dose for too long.

With both Prozinc and Lev, you want to evaluate the dose after a week. If the numbers aren't where you want them, you increase the dose. It looks like Squeaks has been at the current dose for over 2 months.

This begs the question, what is your goal for Squeak? Are you aiming for regulation or remission. If the former, his dose may need a small amount of tweeking to keep things more in the blues and greens. If you want to try for remission, you will need to be more aggressive about dosing so you see a lot more green numbers.

My goal is to definitely have Squeaks go into remission. That makes a lot of sense. I must admit, I did get kind of lax with doing curves, so that's totally my fault. :( So what should I up his dose to and how aggressive should I be? I was loosely doing the SLGS but if I need to be more aggressive than that to achieve remission, I'm game!
