We have a standard format spreadsheet you can copy. You need to have or set up a free google account and then follow these instructions from the Tech Support forum to set it up. http://www.felinediabetes.com/FDMB/viewtopic.php?f=6&t=18207

ETA: The one step the instructions forgets to say, is you need to Share the google doc for "Anyone with the link". That will give us read only access to this great color coded spreadsheet we all use.
