Is it normal for smudge's reading to go from 461 to 493 after the first day of starting on insulin?
 
If this is the first day on Lantus you won't see too much of a result yet.Lantus and Levemir need to build up a shed (a reserve) before the dose becomes fully effective.

Also there isn't really that much of a difference (32) and you don't say when the readings were taken in relation to each other. Glucose meters are only required to be accurate within 20%. When did you take the tests?
 
Well, that's only 2 readings. There is up to 20% variance due to the meter. A difference of 30 points is less than 10% so these are really the same numbers.

It's also quite possible for numbers to not be stable. Lantus is a depot-type of drug. What this means is that a small amount of insulin essentially goes into "storage" (i.e., it goes into a storage shed -- hence our reference to the shed, or the depot). Not all of the insulin you are giving is bioavailable. This is also why you need to give Lantus 5 - 7 days in order to evaluate the effectiveness of the initial dose. See the sticky note on this subject. One of the things using Lantus teaches us is patience. The results of the insulin are not immediate but they are long-lasting.
