Spike in blood sugar after being well managed

My cat was diagnosed back in December. His levels were around 460 at the time. He's a Maine Coon so he's always been large. At the time, he weighed 25 pounds. He lost quite a bit at first due to complete change in diet, but since then, it's been a gradual loss. The past couple months his levels have been around 140-150.

I took him to the vet on May 30 because I panicked after he vomited twice. (Still fairly new to this.) The vet said it was probably just the food he ate and his blood sugar was 149. They ran tests and everything seemed fine.

On Monday, I couldn't get a reading at home, so I took him to the vet. He had also been peeing a lot more than usual and sleeps all the time, so I wanted them to chevk him out. His levels were 251. The vet tech that brought him back out to me said it was good because it was down from the 320 it had been last time. Clearly they didn't read his chart right. So I told him it had been 149 just a few weeks ago. He had also lost an entire pound! He went back to talk to the vet again. When he came back, he told me to increase his insulin from 6u to 6.5u (ProZinc) for 2 weeks then bring him back for a full curve test.

This morning he has vomited 3 times and had bad diarrhea. The last diarrhea had a little bit of blood in it. He's also been prone to diarrhea his whole life (He's 11), so I don't know if I should panic again or not. There was a little bit of food in the first vomit pile, so again, I don't know if this is cause for concern. I'm just worried why his levels went up 100 points in just a few weeks when nothing had changed and he had been doing so well. Am I overreacting and this is normal or do I have reason to be concerned?

Kind of "thinking" of things outloud here ;). To make this easier, let me kind of format my questions :smuggrin:
  1. Has he ever had pancreatitis?
  2. Could you post a picture of his more recent lab work? (Feel free to blackout personal info)
  3. What type of meter are you using? Human or pet.
  4. Are the 140-150 numbers his nadir, pre-shot numbers, or random?
  5. Did Vet check for ketones?
  6. Did they examine his colon?
  7. When is the last time he ate and did he keep it down?\
I'm just worried why his levels went up 100 points in just a few weeks when nothing had changed and he had been doing so well. Am I overreacting and this is normal or do I have reason to be concerned?
To me, this says something is not right and you should listen to your fur momma instincts! If he was mine, I'd be getting him to (possibly) another Vet for a second opinion. Please keep us updated :bighug:
 
Thank you. His name is Schumi (after Michael Schumacher). I had gotten his carrier out when he first woke me because I was deciding whether or not to take him to the vet. After I made this post, he vomited again and then walked over to where his carrier was sitting and got in it. He normally fights me to get in that thing so I knew right away, he was going to the vet. Luckily, there are several different veterinarians in that office, so I was able to see a different one this morning. She immediately ran tests and discovered he has pancreatitis. She said that might be why his blood sugar levels had spiked. They administered fluids and gave him an antibiotic injection. I also have some meds for pain and nausea. I stayed home from work today to monitor him. I kept waking him up about every hour and made him walk and smelled his breath. He's walking fine and his breath is it's normal smell. He finally ate a little bit of chicken breast and has kept that down. I'm still monitoring him closely though. His blood sugar was 354 this morning, but that was right before he was to receive his morning injection. I was scared to give it to him before I knew what was wrong. The vet said to go ahead and administer it and then check again 6 hours later. When I checked this afternoon it was 331, so not a lot of improvement there. I have left a message for the vet and plan to call back in a few minutes since I haven't heard anything yet. But you were right, I needed to trust my instincts. I knew something wasn't right. I would rather pay for an emergency vet visit and them tell me he's fine and I'm paranoid than risk it. She told me this morning that I brought him in just in time. It hadn't escalated yet and hopefully we caught it soon enough to get him back under control.
 
The Cerenia was a big help for my boy. He's recently over his mild pancreatitis flare. Don't understand why Vet's like to give the antibiotic shots of Covenia for no reason :confused:. Anyways, I had lots of help here for my boy and his pancreatitis so I'm sure you will too! Big things are to manage pain and nausea so he can eat and keep getting insulin :cat:

Did the Vet do a Snap fPL or Spec fPL test?
