Do a kitty massage and palpate his entire body, looking for lumps or tenderness. The presence or absence of either of those will provide additional clues. Check for dehydration (see my signature link Secondary Monitoring Tools for explanation, plus some other assessments which may provide clues).

My little Buddy has had a complete blood work up and exam, and everything is within normal ranges for a kitty his age, which is 15+ yrs! :-) So, we've started him on an urgent care canned food, which he loves- thank goodness, in an effort to get some weight back on his bones! I'll keep you all posted. Wish us luck.
 
This was recommended to me by Bengal breeders for a young rescue kitty who has IBD and was very underweight, and I have used it with success for her. It's called Kitty Bloom Wate and it's really nothing but powdered animal fat. My girl has put on weight since I started using it, and I think she must find it fairly tasty as she scarfs her food up without hesitation. You might want to research it, and see if your vet thinks it might help your Buddy. :)
