Cathie....

Her SS is looking really great but we want those preshot numbers down. How about we try a little experiment and we’ll start during the day to see how it affects her PMBG.

About three hours before you would test her for her PMBG, how about feed her a couple tsp of LC food. Let’s see if that brings her PMBG down. Since her pancreas seems to be working after you feed after AMBG/PMBG, let’s try this little experiment and see what she does. Paws crossed we start seeing some green PMBGs. Then we can decide what to do about the AMBG.

If that doesn’t work, then I have a different Plan B. ;););)
 
Woo hoo!!!!!! That's great! I'd do the regular feeding and testing three hours tonight and see if she comes down more.

If you test her and you get a low number, don't worry....she isn't getting any insulin and she might just normally like lower numbers.

The goal is to try and get her to stay in green as long as possible with small meals throughout the day. If we can get her to stay in green with feedings, that's more healing time!!!
 
Shaak may have impaired fasting blood glucose levels - not enough basal insulin secretion between meals or a signalling problem between the pancreas and the liver which results in liver output of glucose outpacing insulin secretion when the cat goes more than a few hours between meals. (Saoirse has had this problem.) As Marje has recommended above, feeding regular small meals throughout the cycle will take advantage of bolus insulin pulses produced by the pancreas in response to incoming food and this should theoretically overcome the impaired fasting BG problem. As long as the next meal is fed while levels are low they should, in theory, stay low.

Based on my experience with Saoirse, next time you need to get blood tests done for Shaak ask the vet to run a panel that includes liver markers, pancreas markers, and cholesterol levels. If the liver values are off a bit talk to your vet about supplementation with a SAM-e and silybin preparation. Saoirse's fasting BG levels improved a lot after she started receiving this type of supplementation. (Samylin at first, then Hepatosyl.)

Yes but I was hoping to see her around 60. But her body doesn't know the difference between 60, 84, or 99.

Impaired fasting glucose is not uncommon in cats in remission. The more we can get her into numbers below 100 and the longer she stays there each day, the better her chances are for a stronger remission. If she was on Lantus or levemir,mshenwojld still be getting insulin to give her pancreas that extra support.
 
I am getting a few hours of sleep in before 4am and her next feeding. Starting tomorrow I will have her pet feeder set up and won't have to get up at 4am to feed her. I am looking forward to tomorrow's numbers.
 
Lol! Here it is almost 1 am and I don't have to be awake to test right now, however, Shaak has decided to play with a bag and jump off and on the bed a few times in sheer happiness. She is absolutely silly with delight! I am gonna need a nap in the morning for sure! I love how she is going through such noticeable changes.
