Scout Gentry 400's

Hello all,

My boy, Scout Gentry, has remained in the 400's most of the week. He went in last Friday and was checked for UTI (all is well). Monday of this week he had his teeth cleaned to remove any chance of bacteria in his body. Checked him several times during the week and his numbers have gone no lower than 411. He is eating well and is alert, although he has neuropathy. HIs dosage is currently at 4.5 units of Lantus 2x daily.
Any thoughts? Do you think we should try another insulin? Not sure what to do....

Good plan to get anything medical checked out.

We would need a lot more information in order to better understand what's going on. First, it sounds like you're home testing. That's wonderful. Do you have a spreadsheet set up? If not, please visit the Tech Support Forum and link a spreadsheet to your signature. We are very numbers driven here and we use a specific protocol to guide dosing decisions. That protocol can be found in the Tight Regulation Protocols starred, sticky note at the top of this Board.

How was Scout Gentry's dose arrived at and how quickly were dose increases made? If you read through the protocol, we make dose changes in small increments so an ideal dose isn't missed. We also test a good deal here. Pre-shot doses are essential as are checks during each of the cycles.
 
Agree with what has been said--Please read the Stickys at the top of the page, they are full of great info and how we proceed here in lantus land-
--There are so many people to help you here...That dose was undoubtedly prescribed by the vet-- Yes there is a weight & dose application, but also, here we usually have a starting dose of 1.0 unit BID and we work up from there--Please post daily, read the info, and ask questions--It is possible that dose is too high & is causing the BG to rise-
We usually test amps, +1, +2, +3, and +6 during the day to see where the bg is going and same at night--I know it is overwhelming, but please take in all the info our members will be giving you..Welcome to lantus land..
 
Checked out and read the Stickys at the top of the page and have a few questions - When Scout Gentry was first diagnosed, I was instructed to gently roll the vial in my palms. The page states not to roll or shake?
I have ketone strips and test Scout Gentry's urine, he is negative. The vet stated I could use the same vial of insulin longer than the manufacturers instructions. Although there was still plenty left in his vial, a new one was purchased on 9/15 just to be certain it was still effective. I changed Scout Gentry's diet to high protein/low carb about a month ago despite the vet advising to feed him Science Diet w/d dry food. He currently gets mostly wet but is a grazer so high protein dry is left for him (and his bro and sis). Do not have a spreadsheet but will work on that today. Scout Gentry's was increased gradually from 1.0 unit in June (when he was diagnosed) to his current doasge of 4.5 units. He is 11 years old and weighs 16 pounds. I am giving him Methyl b12 but without his diabetes being controlled there is no improvement although his back legs seem less shaky. He does urinate on his left foot, which I clean off with a waterless shampoo each time he visits the litter box. I am wondering, can his numbers be elevated due to too much insulin? The last good reading he had was 112 on October 16th. He was on 4.5 units at the time. I believe I am doing everything correctly for him but cant get his numbers down. The vet is also uncertain and advised the next step would be to see a specialist.

Finally, Scout and his brother, Boo Radley are named from my favorite book To Kill A Mockingbird. Just liked the way Scout Gentry sounded. They have a sister named Eliza Jane. Every cat needs a middle name so when their being naughty they know you mean business! :)
 
You don't need to roll Lantus. There are types of insulin that you roll -- they are in suspension and the materials settle. Lantus is a solution and is a fragile solution so no rolling or shaking.

Many people here do get longer than the 28 days recommended by the manufacturer. Many do not. Most of us use the Solostar pen vs. the vial because it's more economical.

It's possible that your cat is getting too much insulin. Without seeing the numbers, it's impossible to know.

Depending on when you started the Methyl-B12, it can take a while to see results. It can take 2 months.

I'm not one of the wise ones here but the consensus here is that it is best to eliminate dry food altogether. Even the so-called prescription ones are LOADED with carbs and many people here see huge elevations in BG just from their kitties eating (or stealing sometimes!) a few mouthfuls. The foods on Janet & Binky's food lists are the most recommended ones. I'm sure others will chime in here but note one thing: When dry foods are eliminated that alone can cause BGs to drop drastically - so it is very important to keep a close watch on BGs, test frequently and make sure you are prepared for whatever happens. If you don't yet have your "Hypo Kit" and 'what to do' instructions printed out, please read the stcky's for details and please do what they suggest - it\'s unlikely you'll ever need the hypo kit with Lantus but still do get one together - just in case!!!

ETA: Nearly forgot to add, didn't see anyone else mention this yet, unless I missed it, but get some Ketostix (in the Diabetic supplies aisle) and test Scout's urine for Ketones (daily if poss) - as with with high BGs, ketosis is a major concern. If you should get a positive Ketone reading, it indicates an immediate vet visit!
