Schroeder is 47 at +8

Figures that 15 minutes before I'm supposed to head out the door for an important engagement that Schroeder would throw me a 47 - his lowest BG yet... Put some food down, and will test again imminently. I will keep food down for him, and will have my neighbour NOT give his 8:00 shot as I had arranged. I will have her come and check on him for sure. I assume that not giving the shot is the right call, but should I give him anything when I get home if his numbers are 100+? Or should I start fresh in the morning? Looks like I have to back him off to 3.5. Not sure what's going on.

Thoughts?

Yes you did the right thing. And we should probably adjust his dose down even more!

Had you done the split shots earlier today?

I'm sorry he gave you a scare. It may be a better idea to drop down to 3U as Sheila suggested earlier this week. Darn, I thought he was holding well at 3.75U, but a seemingly sudden downward trend can happen once they get in good dose territory. very glad you were testing!
 
Assuming you skipped the shot - good. And don't give any when you get home. Just start fresh tomorrow.

I will again suggest the 3u. My thinking before was that there were some low numbers and with all the adjustments and additional insulin, there really wasn't any settle time on a given dose.

For what it is worth, 47 is a perfectly safe number. Both of my guys gave been in the 30s with no ill effects. It's not a number I like to see, but I don't worry about it, just feed and monitor and decrease the dose as needed.

Schroeder may be very high tomorrow morning because of the "new" lower number and skipping the dose. Just give the 3u and plan on it taking a few days to settle again.
 
Thanks for the prompt feedback, ladies. Very appreciated. I didn't start the split dose because i had arranged for my neighbour to come and inject at his normal time. Funny, Sheila, that you should mention not to worry about those low numbers because other than being hungry, Schroeder was showing no other signs of hypo. I will back it (Lev) off tomorrow morning.

At midnight, Shroeder was 346. With 8 hours before his next Lev shot, I gave him 1.5U of Humulin R to get him down for 4 or 5 hours. Should be clear of his system by morning.

Wow, have you tested him on R alone? 1.5U of R is a huge dose in my experience with it. .25U would bring Gandalf down 200 points in an hour. Yes it's in and out fast, but the speed with which it works can cause problems. I worry far more about too much R than too much Levemir.

I understand wanting to get that 346 down and keep it from going to 500+, but again I stress caution with the R. 346 is not an emergency, 300s are going to happen.
 
Yes I've had him on 1UR and 3ULev at the same time and ran numbers on him. Nothing to worry about. He was totally fine all night. 575 this morning as I expected. Gave him 3ULev. I still think that's low for him and I expect to see blacks all week. Are you sure I should be scaling back so far? Even though he dropped so low yesterday, he's still in good numbers 95% of the time.

If you are not comfortable lowering to 3U, then don't hold it very long. Yes doses should be held for 3-5 days, but we also don't want a kitty to remain in reds and blacks for that long. Even at 3U though I don't expect him to stay in the black for that long.

I suspect the 3U will only take a little longer than 4U to bring him down. On the 22nd/23rd 4U took less than 12 hours to bring him off 500 and he was below renal threshold well within 18 hours probably because of the 143 almost 24 hours later. That is actually very fast because the insulin was working against the hormones which probably caused the 500s in the first place. Most of us have seen it take more than 24 hours before a rebound clears. So in Schroeder's case that's one piece of evidence that causes me to say 4U is too much.

I know it's hard to sit on your hands when they run high, but the benefits of holding a dose sometimes take 5 days to reveal themselves.
