Can I have some experts weigh in? I am in unfamiliar territory with the pancreas stuff going on. I'm pretty sad right now and hoping I'll be able to help my poor little guy.

Here is the plan forward so far:
  1. Continue the Wellness canned diet
  2. Start Pepcid (famotidine) – ¼ of a 10 mg tab orally every 12 hrs. We can also dispense it as a transdermal gel
  3. Start Orbax (antibiotic)

You sound like how I felt a few months back when Saoirse had all those scans and tests done. :bighug:

I can't comment on the significance of Rudy's Spec fPL because the lab that did your report uses a different reference range to the lab that does Saoirse's. I do know that from seeing posts here and in my reading elsewhere that just because a Spec fPL result is very high it does not necessarily correlate directly with severity of symptoms. For example, I remember reading here about a cat whose Spec fPL result was more than twice as high as Saoirse's. The other cat was fairly OK at the time but Saoirse was really poorly. For pancreatitis, the IDEXX treatment guidelines are very helpful.

Recommendation: A course of B12 injections can really help with pancreatitis - both in terms of a general boost, but also it can help with appetite as well.

What are Rudy's clinical signs/symptoms like at the moment? Is he nauseated/inappetent? If yes, then ondansetron may help more with nausea than famotidine (though famotidine is good for stomach acid due to CKD issues - both can be part of a treatment plan); and an appy stimulant such as cyproheptadine can help with that (usually only tiny doses are needed - too large a dose and it can make the cat drowsy/depressed).

Saoirse also had low thyroid results. The specialist said that this can be a knock-on effect from the pancreas problem (look up euthyroid sick syndrome).

For the kidney issues, I'd recommend joining the support group at Tanya's Site. There are some very experienced members of the group and they routinely look at lab work for other members. (They're very friendly and helpful, too.)

I hope that's OK for now, Karen. (On new meds here and struggling with side effects.) I'll check in again hopefully tomorrow to see how you're getting on.

Rudy is on B12, he has not been eating much though. I'm really worried about him. I'll check out the other drug you mentioned. Have you had tat antibiotic in particular? That won't do anything bad to him right? I just don't want my poor little one suffering like this. I need a magic wand for all of these kitties!

indicate kidney insuffiuciency

The high potasium
Potassium 5.4 3.7 - 5.2 mmol/L H
is strange since typically kidney insufficiency leads to a low potassium,
These tow values
RBC 6.92 7.12 - 11.46 M/μL L
Hematocrit 31.9 28.2 - 52.7 %
indicate either red blood cells (RBC) are dying early or that not enough RBCs are being created. The later typically happens with kidney insufficiency.
Another good indicator of kidney insufficient is the specific gravity of the urine
 
Saoirse had a course of Stomorgyl 2 antibiotic treatment during her major flare last year. It contains the same ingredient as Flagyl (metronidazole) plus spyramicin. Our vet said it doesn't taste as foul as the Flagyl so is easier for the cat to take. She has never been prescribed Orbax.

I really feel for you, Karen. :bighug: Saoirse was extremely ill during her bad flare last year and it scared the living bejeebers out of me.

Hope some of the above helps. Please keep us updated, Karen, and do please post if you need a bit of moral support with this. I found it incredibly difficult emotionally and I was very grateful to the FDMB members who helped me get through Saoirse's flare last year.

just when i get a handle on the diabetes....sigh....

thank you all for the links and thoughts- for some reason it allows me to cry and let it all out. Then I can suck it up again and be strong for Rudy and my family. I'll grab his antibiotics and pepcid tonight and see how this goes- paws crossed
