Rover - numbers all over the place

Hello. My cat is just shy of 16 and has been a diabetic for 3 1/2 years. Lantus 4IU every 12 hours. In early December, he started drinking more (he has always been a heavy drinker). Vet diagnosed CKD. Treating with subq fluids, aluminum hydroxide, and fortekor.

He eats Wellness grain free wet mostly. Supplemented with Wellcore Core dry, NI dry. He gets a few pieces of K/D dry while getting 100 ml subq fluids 1x daily. He loves it and it helps keep him still. I try to stay as low carb as he will deign to eat.

A few weeks ago, he started showing signs of stomach acid and constipation. Treated with Pepcid AC, then a shot of Cerenia. Didn't help. Now treating with ranitidine 2x daily and miralax 1x daily. Subq fluids increased to 75ml 2x daily because he wasn't drinking enough and the addition of miralax.

We get him feeling better then a few days later he sinks again and we start all over.

Did blood work on the 10th of May. His glucose was 2.1. Fructosamine was 251. The night before, his reading was 1.7. I did not give Lantus, but did give corn syrup. For comparison, fructosamine in December was 408 ("fair"), glucose was 12.6. (I'm sorry, I don't know how to convert to the numbers that other members are posting.)

The vet said that Rover had reverted and to stop Lantus immediately. Two days later he spiked and a different vet at the practice said to give him 2IU 2x daily and watch closely. Did in-house urine testing. Glucose +4.

His numbers have been jumping all over the place. On my Freestyle human glucometre, there have been readings of 1.7 to HI (over 50). I haven't kept a proper journal of readings. I keep thinking I'll remember and then don't. Also, hard to do readings consistently because of work.

Because of the HI reading (and he smelled sweet), I increased his dose to 3 IU 2x daily. Vet said 1 unit increases were okay. I've attempted a spreadsheet with the numbers I have. Hoping to do a curve one day over the long weekend. My glucometre reads slightly lower than the one at the vet clinic.

Thinking that the bottle might be wonky, I started a fresh bottle tonight.

I've had kidney cats before and feel comfortable dealing with that. But I don't know what to do about the swinging glucose numbers. I don't know what I'm looking at. His numbers were stable for 3 1/2 years. Fructosamine always in the "good" range. Any help appreciated. Thank you.

Based on your SS I would drip dose to 2 units since you got lower BGs than on 3. A too high of dose can result in higher BG at least for a while.
Does Rover eat the same about each day, included the same about of dry food. If the amount and type of food changes that can cause erratic BGs
 
He does eat roughly the same amount each day. Is also offered whatever we have for dinner – beef, chicken, turkey. One the days he wasn't feeling well, it was more of a challenge and we resorted to FF chicken pate.

Interesting about a high dose resulting in a higher BG.
