You might want to check that Rom's not going even lower further on in the cycle, Sue; he does seem to have a bit of a predilection for unusually late Caninsulin nadirs. Also watch out for any potential carryover on the new dose.

he was 16.0 (288). And he hasn't eaten!!!
Could be bouncing back up off the lower value earlier.

I know that this may seem counterintuitive but I've found that one factor which may influence BG spikes is too long a period of fasting (in cats with impaired fasting BG levels).
If Rom's pancreas does actually pulse some insulin at mealtimes it could help keep his numbers in a better range over the course of the cycle. How many hours does he normally go between feeds and how does that compare to his feeding pattern today?

He usually gets fed at 6.30am and pm and then about 11.30 am and pm. There was no difference today.
