Reccomendations on Lantus Level

I did a curve on Caesar yesterday. Could someone please check out my spreadsheet and advise me on Caesar's Lantus dosage? June 8 was the last time the vet requested that I raise his dosage from 1 unit once a day to 1 unit 2x a day, which we are doing now. He is consistently in the high 300-low 400 range. Doesn't it seem that he needs more? Thank you in advance.

There are a few things that will help us to lend you a hand with dosing. First, Lantus is a twice a day injection in cats. Cats metabolism is much faster than a humans and as a result, you need to give a shot twice a day. It's good that you are now on a twice a day schedule.

With Lantus, dose is based on the lowest point in the cycle (i.e., the nadir). Functionally, what that means is that you need to get at least one spot check during each shot cycle in addition to your pre-shot tests. You've been doing pretty well at getting tests during the AM cycle. You need to get at least a test before you go to bed each night, as well. There are a couple of reasons for this. First, many cats experience lower numbers at night. You don't want to miss catching those numbers since it may mean a dose adjustment. In addition, if you don't test at night, you're missing half of your data. It is extremely difficult to get the full picture of what's going on if half the data isn't there. We're all guessing.

I do suspect that Caesar needs a dose increase. Would it be possible for you to get a test or two this evening?

I don't have advice, but based on my vet's recommendations with my kitty (similar numbers, but he's had no change in his symptoms - still flooding the litterbox), his dose was increased today by .5u. I wonder if your vet will do the same thing.

Thank you for your response. My AM tests are actually done between 2pm and 4 am. I work nights, so I am not on everyone's day schedule. So basically my curve or extra tests are being run between those times- both in the day in in the middle of the night. I hope that info helps.

That information helps -- a lot!! (You might want to put a note that you work nights in your signature so we don't keep bugging you about it. I'll admit -- I'm notorious for forgetting those kinds of details.)

One think that many of us who are not typically home for half of the day try to do is to get tests when we can. We test before we leave the house and/or when we come home. The more random spots you can fill in on your spreadsheet, the better. In many ways, getting those random tests is better than a curve. Not all cats have their nadir at +6. My cat, for example, typically has an early nadir (around +4). In addition, the nadir can shift. Just do the best you can so you don't end up feeling like you're making a dose change without having some idea of where the numbers are falling. The overriding issue is to keep your cat safe and the only way to do that is to know where a dose is taking the numbers.

If it's possible to get a test in, either on a day off or whenever you can, if they are consistent with the AM cycle, then I'd increase.

Have you had a chance to look over the starred, sticky notes at the top of the board? The info on the Tight Regulation Protocol will give you the basics of how most of the people here use Lantus. You do have the option of the Start Low Go Slow approach, as well.
