Yay for negative ketones! You've just missed Dyana. I don't have personal experience with R just what I've watched on the board. You don't want to change the Lantus dose yet, you need to hold for a minimum of 3 days (6 cycles) so I assume it is the R you are wondering about. I would think that if you are home to monitor you could do the same as yesterday and give .25 of R with the lantus dose. I know it doesn't look like it touched the sides much but it does look like he is feeling better with negative ketones and eating better so I'm not sure that you want to use more this cycle - especially not if there is no one here with the R experience to tell you to do so.

Good Morning, Tara! That's great on the ketones :-D

For this morning, go ahead and give her the 3 Units of Lantus, and then after that, give her 0.25 of R, as long as Sasha is eating and seems to be feeling fine. How are you holding up?

Dyana, I passed out last night but woke up to take his BG at 2 am and feel back to sleep again- i was soooo tired!!!. I am feeling much better today with sleep!!! And seeing Sasha well is making me so happy :) Thank you so much!!!!

Tara, welcome and I just wanted to lend my support. I just read your other thread, and you have done a great and amazing job with Sasha. The Beans here are so knowledgeable and amazing. There are a lot of us cheerleading you on with Sasha, but just not wanting to get in the way of your getting help. You haven taken in a lot so far. I hope you were able to get some rest. Lots of love and vines to you and Sasha.

Tara, I have to leave, soon for work. You'll still need to keep testing for ketones for awhile (a few weeks) and keep Sasha eating, and we need to keep working on getting the blood sugar down. Please get those hourly tests while he's on the R, just in case he responds more today, than he did yesterday. I wish you both a great day.

Please remember to get +1, +2, +3 and +4 with the R.

Just an FYI - if you put anything other than a # in the cells of the SS, you lose the color coding. If you look along the top line, there is a tiny black box with an "A" in it. You can use that to manually color code the cells on the SS.

Hope everything is okay nailbite_smile it is especially important to get those tests with the R on board, you can't rely on things working out the same today as they did yesterday.

Hi every one, thank you for checking in with me. Even from Australia! Wow!!! .... I am so sorry.. I have been awake but doing work from home, I have been testing Sasha and I am concerned. His BG's are still high, even with the .25 R and 3 units Lantus, (he was previously on 2 units lantus but the vet bumped it up because he got ketones again this week) so his numbers today are AMPS 364, +1 399, +2 381, +3 372, +4 365.

He was very spoiled yesterday eating all the high carb fancy feast with gravy, and I have been mixing it with his Wellness to wean him off. But started trying to give him Wellness on its own when I saw the high BG's and he's refusing it now unless its the FF with Gravy! Tried to spruce it up with oregano, bonito flakes, parmesan cheese... he's not having it! So I just gave in and gave him a mix of FF with wellness again.

I am wondering if maybe when I measured the R I gave less than the .25 I thought, he's definitley not eating as much high carb food as yesterday because I've been giving him less, or if this high BG is a reaction from bumping up his Lantus so quick?

He's still Ketone negative, thankfully... but he's one kitty full of sugar!!! I'd like to get him down...

Regarding the food...please stick to the HC food for now, don't try to switch him back to LC yet. The last thing we want right now is for Sasha to slow down or stop eating. If you didn't see Sienne's post toward the end of yesterday's condo, I'm pasting it below...

  • The good news is the ketone levels are coming down. DO NOT become complacent. Ketone levels can rise quickly. Please keep on your toes and doing what you've been doing (because it's working!!).
  • Test for ketones whenever you can. This is the best early warning system you've got.
  • Stick with what you've been feeding. Right now, you're balancing all of the insulin you're giving with food. If you lower the carbs, you may need to lower the insulin and that's not a good idea at the moment. (When Desi's Tawny was dealing with the same issues as Sasha, I told her I didn't care if she fed Tawny a hot fudge sundae. Calories help to combat ketones.)
  • Keep being as careful as you're being so you don't confuse which insulin is which. If you're not sure, start over. Wasting a syringe that's filled with insulin is far less expensive than shooting too much R.
  • Know that there are a lot of people here to support you. Many people may not be posting in your thread because they want to make sure you are getting the information you need at a critical time. If you need anything, ask.

As for the vets, I would strongly suggest you get a copy of Sasha's records from the vet hospital. In particular, get the ultrasound report, lab tests, and the notes from Sasha's hospitalization. I'm not comfortable with someone saying that there is a kidney infection and then saying, "Oops! My bad. No infection." Have the information in your hands if for no other reason than if you change vets, you have Sasha's most recent tests. Baytril is not a trivial antibiotic and antibiotics should not be given, "just in case." That's a great way to cause antibiotic resistant strains of bacteria.

Most vets haven't a clue about whether home testing stresses a cat. The reason is that most people don't home test. The vets don't have a baseline. On the other hand, if people don't home test, they have to go to the vet for curves and fructosamine levels and the vet makes money.

You've been doing great. Please do try to get some rest. Hopefully, Sasha has turned a corner but there's still a way to do.

Thank you so very much. He is eating his Fancy Feast, gobbling it down!!! I have a lot to learn, and I came to a great group of teachers. I am on top of him today for sure!! I am just wondering if he should get some more R today? And fluids?

I will be getting all of that paperwork from the vets for sure!!!

Tara, I asked for some R help for you....I don't have R experience, and it's best to leave that to the people who do. I suspect that you wouldn't want to give more until at least +6, but that's just a guess on my part based on past advice.

I'm hoping Marje will have time to help with the fluids. Are all the fluids absorbed from the last dose?

Laurie, you actually are a lot of help!!! I am going to keep him on the high carb food... do everything the same as yesterday and hold off on the R until I get a "go ahead"!!! I got nervous when I saw his high BGs and when I tried to give him his Wellness...I swear he looked at me like I was crazy!!!!! :lol: He's loving this Fancy Feast!!!!

You will be okay to follow the same advice from yesterday and give another dose at +7 if Sasha is still pink and you can continue to monitor hourly. You should only go with the 0.25 dose again unless someone with R experience comes on in the meantime and advises you it is okay to do more. It is after midnight here now though so I am going to bed. Do you know what to do if per chance his numbers plummet?

Tara....doing great here! Couldnyoumplease do the total body check on him and make sure there are no squishy parts or pockets of fluid and if you are weighing him like I suggested, then please do so to make sure he is not retaining fluid.

If there are no fluid pockets are squishy areas, why don't you give him 75 mls of fluid twice today separate from the "R" duration time and let's see how he does. If you see any trace of ketones coming back, we'll increase it again but the fluids will help him overall and hopefully are helping a tad with the BG. Are you doing ok with the fluid part...no problems?

Actually...no offense to Serryn...but you have only given 0.1u of R at/after +6. I would not assume that it is ok to give 0.25u at that time. That's why I said to wait for folks with R experience...it's potent insulin and needs to be dosed carefully. I've watched many people use R, but without personal experience, I am not comfortable advising anyone on it's use.

No offense taken at all. I was thinking that the dose was upped to 0.25 last night because the 0.1 wasn't touching the sides. The R from earlier will have worn off completely and Sashas ss to date doesn't show any late drop from Lantus. The circumstances are still the same. Hopefully someone that does have R experience will come along soon and confirm or deny. I am heading for bed - goodnight ladies :smile:

Ok, I am going to go ahead and give him .75 fluids twice today. There's no lumps or bulges on him at all so I think we're good there! Ok, I'll hold out on the R. I have to go to the store real quick and get more Fancy Feast because he's pigging out... he looks great but he's filling up that litter box, so let me get more fluids in him now. he is still ketone negative though! Serryn, R dosing is not as simple as just giving the same amount each time. Different doses are often used depending on when it is given. I have not seen anyone with R experience suggest to Tara that 0.25u be given at or after +6. The suggestion *may* be to give 0.25u, but I would not *assume* that based on what was suggested for PS.

I'm glad he's eating and still negative for ketones! Thank you Marje for stopping by to help with the fluids!

For the next R dose, how about trying 0.5u at +9, numbers permitting. That would be 3pm eastern, correct?

exactly how many oz. of food do you think Sasha has eaten today? Appetite still good?

Hmm this is weird, he went up again. No food for 2 hours Pre shot, and he was sleeping any way. Just fed him again and gave him 3 units of Lantus. Tested him, he's ketone negative!!!

YAY on the negative ketones!!!! You might want to check his fluid saturation with the physical check and if he's ok, then you could give him 75mls of fluid between L+2 and +4...does that sound good?

Garland kinda does the same thing with R. Even though its a strong insulin she never "dove" down with it. She would kinda stay flat, then rise. It took a few days for her numbers to come down while using it. Eventually the R would help the Lantus "catch" the lows. (If that makes any sense) So it wasnt an immediate thing for us when we used the R, and we had to up the amount. But you do have to watch closely no matter what, because every cat is different. :smile:

He's coming down a bit! :smile: and just tested ketone negative!! YAY!!!! he's got a little pot belly when he lays down, i guess from eating so much...it's so cute lol. I would love to see him put weight on... the last couple of weeks he looks like a little bag of bones...just gave him .75 subqs!
