Question about switching insulin.

Hello everyone it' been awhile since I posted here. Frisky is doing well but the other day I get a call from her vet and she tells that the insulin(PZI(Prozinc)) is on back order and they have no idea when they will get it in, So frisky is going to be switching to Lantus on Monday morning.

My question is. Should I skip her Sunday night shot or go ahead and give her the prozinc Sunday night and just switch to the lantus on Monday Morning.

She was on 4u twice a day(u-40 syringe) and now will be starting on Lantus 1u twice a day(u-100 syringe) until we get her back to normal. I am on vacation this week so I will be able to keep an eye on her.

The PZI is usually cleared out of the system within 12 hours, so you should not need to skip any shots.

Watch Frisky's blood sugar closely during the first week and also check urine for ketones. Ketones are a risk whenever you make a dose decrease. If the ketones get too high, hospitalization is required.

If ketones start to show up --- you need to increase insulin more rapidly than the usual "start low, go slow" approach.

Phoebe is correct. ProZinc clears the system and you're good to start the next cycle. When I transitioned from ProZinc to Levemir, I started at 1/2 the dose Grayson was on. It's REALLY important to check for the ketones when you do this, as you don't have the freedom to adjust the dosage as easily as you did with PZI.

I just saw your last paragraph. Keep up the testing, and you should be okay - just stay on top of anything out of the ordinary. Good luck with the transition.
