Yes you would start on 5 units of Lantus - though honestly, with that size of dose, Levemir would be a better choice. Lantus has an acid base, which can sting at higher doses for many cats, and I rather suspect Blitzey will end up needing more than 5 units. However, let's see how it goes and hope Blitzey is fine with the Lantus.
Can you tell me a little bit about Blitzey's history, how he was diagnosed, any other medical issues? Good to see you are on top of his ketone testing with him in those high numbers.
Dr Elsey's dry, He eats very little, he is on cerenia and sub q fluids as needed, just came off antibiotics(orbax)Thanks for the update on the signature. Is he on any medications for any of those conditions? Some medications can contribute to needing higher doses.

It's hard to predict what acros will do regarding dose. We've seen them on as little as 4 units (I saw one study that said they saw 1 unit as the low) and as high as "you don't want to know". Well into double digits for sure. Levemir would definitely be a better choice and I'd look to buy in from Marks in Canada for price savings. There is someone on the Supply Closet forum who sells it regularly too.
