Hello everyone,

I've been successful in completing Pixie's spreadsheet and posting it in my signature. At Julie and Pumkin's advice, I am posting Pixie's spreadsheet to this group even though I have followed a slower approach to dosing as per vet advice. Perhaps you will recommend that I post to the slow approach with Lantus group. Please know that I am not adverse to using whichever protocol that will make Pixie well again. All Pixie's information is in the health forum and I don't know whether this needs to be done again. I'm sure someone will let me know.

I have a few comments about the spreadsheet because as I was filling it in, I couldn't believe how coherent everything became. Please note that the vet did not want me testing outside of the BG curve days, except if I thought Pixie might be hypoglycaemic. I really didn't feel comfortable with this as it felt that I was operating in the dark and that's not a comfortable thing for me. So I tested but not as much as I wish I had after reading here at FDMB.

The first couple of weeks after the DM dx, I was giving Pixie 1 unit and he seemed to get better the first two days and then went downhill from there. So I decided to ask the vet to do the blood test for pancreatitis and this took about a week to get the results. At this point, Pixie was at death's door and the vet phoned and indicated that Pixie had pancreatitis and he would consult with a specialist. This was when Pixie was started on the pancreatic protocol he is currently on and the consultant wanted us to go back to 0.5 units bid as she was worried about a somogyi effect. Pixie improved but he had up and down days. I completed the BG curves as the vet asked after about 7 to 10 days on a certain level of insulin. When the graphs were sent to the vet, he would decide whether to increase the dose. In the beginning we increased by 0.5 up to 2 units and then jumped to 3 and then 3.5 and 4.

I believe, after reading the lantus protocols, we may increased by too much and may have overshot Pixie's optimum level. This might explain why his numbers got higher and higher between doses instead of getting lower and lower. Maybe he was getting too much insulin and his body was responding and he seemed to go up and down. I'm not sure but I'm really interested in what everyone thinks.

I'm glad to see you made your way over to the TR forum! I don't know if you saw my response to your thread on the Main Board:

I clicked on your spreadsheet but wasn't able to access it. I think we need to do just one more thing with it. When you log in to your Google account and pull up your spreadsheet, there should be a blue "share" link on the upper right-hand side of the page. Click on that and select the option to "share with anyone who has the link." That will allow us to see it.

Thanks for the heads up Sienne. I had the wrong template shared and didn't realize I had to change it in my signature. Hopefully it works now and if it doesn't, please let me know. Anne
 
I can read the spreadsheet now. Yay! There are two tabs, one for US numbers and one for world numbers. You will want to enter numbers in the world tab and the US numbers will be automatically calculated. (I think you entered world numbers in the US tab) You may need a blank spreadsheet to get this to work correctly. You are doing a super job with everything. It's a lot! :-D

Hi Anne and welcome to you and Pixie from a fellow Canadian. You can look at Neko's SS and see that we have two tabs. I enter numbers on the World SS and it automatically converts it to the US format.

Following up some comments from your Health post. I have not been able to find suitable all meat baby food in Canada. The one mentioned by Elise is not available here. I just did a cross border shopping trip today for cheaper test strips and got some of the baby food. You want something that is just meat and meat broth. Most of the Canadian stuff has vegetables and hence too high in carbs, or they contain wheat (gravy) which is also carbs.

My Neko is on daily buprenorphine for pain associated with her acromegaly. I get a written prescription from the vet and then get it filled at a compounding pharmacy. It has a shelf life for 3 months. I also get it chicken flavour, although they also make it plain. You might want to try that instead of relying on the vet as the middle person for a supplier. Since it's a controlled substance, at least in BC you need a new prescription each time you get it filled.

You commented that Pixie's numbers are increasing as you increase the dose. There are a few reasons for that. You might want to read the post on New Dose Wonkiness. Both it and the second part of that most which describes bouncing are two explanations for higher numbers. The other reason I suspect you have higher numbers is the food you are feeding. There are off the shelf products with unique proteins that are lower carb foods. The Sensitivity and Hypoallergenic foods both have rice in them which will make them higher carb. Nature's Variety and Ziwipeak are two examples of manufacturers of unique protein wet food that are low carb, and will be cheaper and better quality than the veterinary food.
 
i'm seeing the same thing that Liz mentioned - go to the world tab on Pixie's spreadsheet and enter the numbers there. I'm not sure if you can do that and have the numbers appear on the US tab as they should or if you'll have to remove the numbers from the US page first.

You might be able to do it one line at a time. Look at Feb 25th on the US tab, write them down, delete those numbers from the cells then enter them into the world tab on 2/25. Go back and forth between the tabs doing that.

The world tab has formatting in the cells that make it so that you enter the mmol in those cells and it translates them automatically into US numbers on the other tab.

If that is too cumbersome, it might be easier to start a new one. Sorry for the goof-up - i didn't even think to mention the different tabs.

thanks for doing all that work! I can see that Pixie had a nice blue nadir (low point) in his cycle during the day today. That's great.

My guess is that he's needing a higher than typical dose of insulin because of the food. As I said yesterday, that exact food isn't listed on Dr. Lisa's site - all of the Royal Canin foods were the US versions. Here is the Royal Canin Sensitivity VR formula. It doesn't tell how many carbs are in it - i think if i were you, I would contact the company and ask them for the AS FED carb count. I compared the ingredients to all the US products and the ones in this formula are different. The prime ingredient in the Sensitivity VR formula is venison, which is not in any of the US products. Fortunately it doesn't have wheat or other grains in it, so it may not be as high carb as i was originally thinking. So . . .

It would be helpful to know what that carb count is, however, if he needs to stay on it. What's the reason the vet has him on this food?

I'm curious why there's an adorable little dog in the picture - it was the only listing on their site I could find for that food, but I'm assuming you've got cat food. There is rice listed above, although I have no idea what Brewer's Rice is - i read through that initially and my brain read "Brewer's Yeast." Rice does increase the carb content.
 
There's an easier way to fix the spreadsheet. Go to the US tab, select the all the cells starting with the first cell on the left all the way to the last one on the bottom right, then do a Control-X. Go to the World tab, place your cursor on the top left cell and type Control V for paste.

Julie - here is a definition of brewers rice - it's still rice/carbs, and too high in the ingredient list:
Brewer’s rice is small fragments of rice kernels that have been separated from the larger kernels of milled rice and can be found in some dog and cat foods. This type of rice was traditionally used for brewing alcohol and is where the name ‘brewer’s rice’ originated from.
Brewer’s rice- also known as broken rice- is an easily digested carbohydrate that contains fiber.
 
yikes - it does sound like an ingredient not good for our sweet kitties.

i thought it might eradicate the cell formatting if you copied and pasted. that would sure be a lot easier if it doesn't.

I didn't realize I was in the wrong spreadsheet. I wondered why the numbers were still showing up in the mmol/l fashion as opposed to mg/dl. Anyhow the problem has been rectified and my numbers are now all plugged into the world template. I like how it color codes the results making it easier to read. Ingenious!! I did this rather quickly so hopefully all the numbers went into the right slots. I'll double check tomorrow when my eyes are not so cross-eyed.

I'm a little concerned with Pixie's number this evening. I lowered his dose to 3 units thinking everything would be safe and I could get to bed early and voila he pops a 5.9 at 10pm and he has about 2 hours to go before nadir. This is like playing russian roulette even though I can't say that I've played this game thankfully. He seems fine but I would be happier if he would eat a bit.

This may be another long night but my husband said he would take a shift if necessary. Wish me luck, Anne. nailbite_smile
