Peeing outside the box

Pepe's blood glucose has been good and he hasn't peed on the floor since ....I think the beginning of the month. I get up this morning and there he had peed by the box. There was a lot of clumps in the box (I cleaned it out before I went to bed) I didn't notice this until after Pepe had eaten but his BG then was only 126. I had thought that maybe all the times before he peed outside the box that it was because his BG had been up. Any ideas what else may be the problem? He hasn't had to have an shot since last week.

Yes as normal as can be for him. He's been checked other times when he pees around but didn't have an infection. And he looks guilty when I see it.
 
It can happen when they're constipated too........are the poops in the litterbox fairly ample looking or dry and shriveled? Sorry for the descriptions (They can actually push so hard trying to have a movement that the pee happens too)
 
No there was only pee in that litter box. I have 2 for him. One in the living room (because he was peeing on the floor in there) and one in the utility room. There was no pee on the floor this morning.
 
Multiple small pees suggest bladder irritation of some sort.

It is possible to have an interstitial cystitis of non-infectious origin.
Some cats with this problem do well on glucosamin/chondroitin supplements.

Food allergy may indirectly result in urinary tract issues. Have you changed foods al all or rotate through varieties? Do any of them have grain? Beef?

He could be developing an infection and it hasn't gotten bad enough to affect the glucose yet

Maybe add notes on your spreadsheet about food & water consumption for several days & see if a pattern emerges.
