My sugar boy Simon has been OTJ for a week. His pre-shots are generally over 100 and under 140 although we have had a couple that were over 140 and after feeding and retesting, they dropped so I did not shoot.

Now, today he had a higher than usual reading of 149 this morning. Fed and waited a half an hour and he was at 134. After he polished off half a can of FF classics and three hours had passed since his AMPS test, I just retested (twice to make sure) and he was at 77 and 80!

Food is bringing him down to a good green number but his pre shots are still all over 100 except for one reading of 90. I do NOT want to give him any insulin with numbers like 80 after he eats!

I tried the smaller dose thing for a while, but his +4 and +6 readings were always in the mid 100's (between 130 and 180) whenever I gave him insulin!

Why is he so high at his pre shot numbers (between 100 and 140 generally and average of 120-125) but he drops so nicely after he eats? I leave food for him all the time so he can nibble and he stays in lovely green numbers but rises around pre shot time.

Am I doing the right thing by keeping him OTJ? I am concerned about the higher pre shot numbers. I was hoping his pancreas would rise to the occasion and stop with the pre shot numbers over 100 after the week he has been OTJ (it has been 8 days since his last shot)

Any advice?

You are on Day 8 of the 14 required, yes? I want you to think of it as he is getting his sea legs back.

I would not worry about the higher numbers at ps time because his body and pancreas are quite capable of intaking the food and producing insulin, then adjusting as needed, and that's what you want to see.

What were your cat's numbers at ps time before you knew about feline diabetes? You don't know. So how do you know if what you are seeing now is what he was doing before his diagnosis?

I would not be concerned if he is able to adjust with his food. Now, feline diabetes is not the only thing to raise numbers, so don't be too quick to jump back on insulin.... illness and stress can raise BG, and even a bad tooth can cause higher numbers, but if food brings him down to nice numbers under 100, I would not worry.

Good luck on the next 6 days!

I agree with Gayle; food is bringing him down into some nice numbers. When you test him and get those high numbers, how long has it usually been since he's eaten? You might want to try feeding him more often if you're not already. This helps the pancreas work better and more consistently at bringing those numbers down.

I think most folks would agree with the advice given here. If food is bringing the numbers down, the pancreas is working.
Keep the faith and continue to monitor. May your OTJ trial succeed! :-D

I am feeding him pretty much on demand. Thanks for all of the advice. I will stay the course and continue to keep him OTJ. I am hopeful that the lovely numbers I see during the course of the day will spill over into his pre-shot (not the right name but you know what I mean) numbers as well. His morning number is highest as he tends to sleep through the night and not eat any of the food I leave for him but if he is sleepy, can't make him get up and eat

I think he is on his way to (not saying it - don't want to jinx it!) not needing any more insulin, at least for now. He is doing great!!
