One more...General Lantus Dose Reduction Question

I've been mulling through a few dose reduction questions in my mind and thought I'd ask for some input.
Question #1: Reducing the dose:
  • If kitty drops below 40 (long term diabetic) or 50 (newly diagnosed diabetic) reduce the dose by 0.25 unit. If kitty has a history of not holding reductions well or if reductions are close together... sneak the dose down by shaving the dose rather than reducing by a full quarter unit. Alternatively, at each newly reduced dose... try to make sure kitty maintains numbers in the normal range for seven days before reducing the dose further.
The part I underlined is really where the question comes in. When does this apply (it sounds like it's saying if you take a reduction, wait 7 days before another one)? It seems like when a cat starts down the reduction ladder, it's often happens right on top of each other, yielding multiple reductions in a week's time.

Question #2: Shooting through a reduction: (I've searched through the Sticky's because I know I've seen this somewhere....couldn't find, so thought I'd go ahead and ask.)

If a cat earns a dose reduction during the morning cycle...what are the key determining factors that dictate whether to shoot through the PM with the normal dose and take the reduction in the morning, or whether to take the reduction on that PM dose. (I would think the PMPS number....the cats bouncing history, etc. but wonder if there is a hard fast rule).

Question 1:
The part that may be causing you confusion is that the sentence provides an alternative. An alternative strategy to a cat dropping below the 40 or 50 mark is for the cat to stay in mostly normal numbers for a week. Take a look at Kathy/Kitty's SS for an example.

Question 2:
There is no hard and fast rule about shooting through a bounce. It's an option that allows you to help bring the higher numbers down.
 
OK...I lied...one more question :roll: :roll:

Is there a limit to the number of reductions you can take in a 24 hour period....specifically, can you take two if you drop below the reduction number on both cycles, or do you assume the second cycle was left over shed action....allowing for only one reduction per day?

PS....I'm chucking as I ask DOSE REDUCTION questions....

Generally, only one reduction since back-to-back reductions seldom hold. Usually, the second drop is as you noted -- it's due to a shed that hasn't caught up to the reduced dose.

There are exceptions. We just ran into this with Stella yesterday. There are times when a cat has hit a breakthrough dose and may need to come down the dose ladder quickly.
