Are you testing for keytones?
I still think his dose needs reducing (whilst monitoring for keytones) these swings of the best part of 300 points can't be making him feel good.
He could be dropping too low, so as well as the fact the insulin has worn off by +12 you have rebound going on (body has released glucagon to raise bg's itself-lifesaving response)
