Non-diabetic refusing to eat + constipation 3x vets later

I haven't been on here in a while but I wanted to ask if anyone has any ideas/advice. This is about Jerry, who is 8 years old and does not have diabetes. He is completely crazy about food, and will eat anything and everything.

2 weeks ago Jerry (non diabetic) threw up about 10 times in 5 hours. He continued to vomit. We thought he may have had a blockage. We took him to the emergency vet. They were convinced it was a diabetic crisis, and me knowing what a diabetic crisis is insisted it is not. I insisted they do an x-ray, and signed paperwork saying I wanted one, and they said no need, and concentrated on potential diabetes. Anyways, they did all the testing and he's not diabetic. They palpated him and said he had some stool packed up. They said he has hyperthyroidism and is mildly dehydrated, and doesn't have chronic kidney disease, and sent us home after giving him Cerenia and fluids.

We went to our local vet the day after, who tested him and said he does not have hyperthyroidism, and has very early chronic kidney disease. No mention of an x-ray again, and they said he definitely does not have a blockage or anything that would indicate an x-ray. They did a urinalysis and confirmed this and did a full CBC. They gave him fluids and palpated his stool loose and gave him Cerenia again. He came home and we started giving him more wet food to get him more water.

He was fine for about 2 weeks. We got a kitten Monday (I have no idea if this matters), and Jerry ate with the kitten 2-3 times, acting normal. About 2 days after the kitten, he threw up one single time, with a slight pink hue (could have been from getting into kitten food). He started acting like he did 2 weeks before, but not vomiting. We took him to the vet for a 3rd time, and they did another CBC - everything's fine, kidneys look better, urinalysis clear, still no diabetes, etc. He is severely constipated. They do an enema and he has a massive movement they say.

Anyways, this was 3 days ago. He is not eating. He is on lactulose for constipation, cerenia for nausea, mirataz for appetite, and he will not touch a bite of anything. He is normally food crazy as I said. We've tried 6 different foods, different textures, varieties, etc. We are now force feeding him high calorie food with a syringe and giving him these 3x medications to no effect. The vet labeled the problem as "constipation". I can't get any answers regarding how long this will go on, what's happening to him, why it's happening, or really how to fix it. Nothing at all. I'm reading and hearing about fatty liver issues if he doesn't eat, and right now he's eating probably 9-27mL of food a day. He's 13lbs. This can't go on for another 2-3 days without consequences I'm afraid, and I'm not getting help from the vet(s).

I think that's all of it.

In short: he's lethargic, not eating at all, was severely constipated and nauseous. He has early CKD and mild dehydration. I can't get any kind of treatment plan or diagnosis or prognosis from my vet besides "it's probably something weird neurological". An x-ray has not been asked for or done. I don't have infinite funds but we're at $1400 and the only useful thing they've done so far I think is the enema. I'm getting very worried and frustrated.

This sounds to me like his GI system needs more exploration, maybe an ultrasound. With both vomiting and constipation, something seems to be happening down there. Thickened bowels could cause both. One possibility is IBD. Was the kitten food a different protein (meat type) than his regular food? Is he still constipated?

Any chance you could get some ondanstron, I find it's much better med for nausea than Cerenia. It will need a script from the vet and you get it at a regular pharmacy.

He had a few bites of the kitten food, but no it was the same type. He's not gone to the bathroom since we got home so no idea. He hasn't ate and has drank a little bit. I haven't seem him go.

I'll ask tomorrow for a few things.. ondansetron for starters and I was gonna ask for x-ray/pancreatitis spot test/some pain medicine for him. I don't like the cerenia at all - I mean it doesn't seem to work and it also makes him froth horribly as it must taste awful.
 
Cerenia is bitter tasting, best given in a blank gel cap if you can.. Cerenia does help with vomiting and doesn't need to be given as often as ondansetron. You can give both as they target different nausea receptors.

X-rays can be a start, but an ultrasound may also be needed.
 
I will say he's only vomited once this week so IDK the goal of the cerenia I think is to not be nauseous so he can eat, but he's not doing that at all.
 
If you hide the Cerenia (and any other pill) in a Pill Pocket or Churu treat or something, there's less foamy "icky taste". Cerenia is also available as an injectable. It supposedly stings though.

If you think an x ray might be useful, ask the vet to do it. Be your cat's voice and insist on diagnostics. Don't settle for a vet being 🤷‍♂️ and dismissing your concerns. My two cents :)
