New Member - Misty - Looking for advice on how to adjust dose

Hi,

I’m Misty, a 13-year-old Nebelung who was diagnosed with diabetes in December 2024.

I was initially prescribed Prozinc, but after being hospitalized for DKA in July 2025, my vet switched me to Lantus. Because I get very anxious at the vet, instead of in-clinic glucose curves, we’ve been doing fructosamine tests every few weeks and adjusting insulin based on those results.

Unfortunately, this process has been slow, and my diabetes hasn’t been well regulated. To get better control, we recently started home testing with an AlphaTrak meter after reading about it on this forum. We now have several weeks of data (spreadsheet) — mostly pre-shot readings, with some additional random post shot readings — and are hoping to start adjusting insulin more efficiently.

My blood glucose has generally been high and fairly flat, staying in the low 300s. Looking for advice on adjusting my dose.

Welcome to the FDMB! I had to look up Nebelung on the internet because I'm unfamiliar with the breed: they're beautiful! I can't wait to see a picture of Misty.

You've already done a lot of work getting your spreadsheet set up and getting your home testing underway! That's great!! Also, you might have already read this information, but here are the guidelines on the two dosing methods most of us on the board follow: Thread 'Dosing Methods: Start Low, Go Slow (SLGS) & Tight Regulation (TR)'. Deciding on a dosing method is important as the methods have specific guidelines in terms of how/when we take dosing increases/decreases. One note: If you are feeding only wet food and can test at least two times in a cycle (the preshot plus one other test), you can follow Tight Regulation, which is a dosing method that has research supporting its ability to get kitties into remission (though, Jude went into remission on SLGS).

Reading through the information at the top of the Lantus board is also something you will want to do, if you haven't already: Lantus / Levemir / Biosimilars

You're doing a great job so far getting Misty everything she needs to get better!
