Hello everyone. My cat Garcia was diagnosed with diabetes last December . He was put on Lantus by his vet that admitted in January he didn’t know so much about this disease to treat him . I value his honesty and I am still seeing him as he is willing to learn and has been very kind to Garcia and my dog too. Said that, Garcia was in remission for some time in April until he got an ear infection . His numbers are very high and I haven’t been able to lower them . Any advice is welcomed .

Hi Patricia

Dropping his dose back to 1u is what we call doing a “rebound check”. Based on the other SS you had, that is hallmark advice from a FB group who still hasn’t learned rebound checks never work and put kitty at risk with high numbers.

But with Lantus, it is not a good idea to change the dose as much as you have been. Lantus is a depot insulin so it takes time to fill the depot and have it start working better.

Were you doing any specific method of regulation like Tight Regulation or Start Low Go Slow? My suggestion is to do TR and we should consider increasing his dose by 0.5u after every four cycles for a few cycles, then give him a break, then do it again until we start seeing better BGs.
 
I have been trying to read all the information . I have been testing him pre shot or sometimes looking for nadir numbers . I have never tested ketones . He has been on 1 dose of Lantus fur more than 7 days so I increased it to 1,50. Should I start testing him on Ketones ? He is on a home cooked diet prescribed by a nutritionist vet. He can’t tolerate raw food .

You should always check the preshot (PS) and at least 2-3 other times during the cycle. Of course, at night when he’s high, if you just get the PS, +2, and before bed, that should be ok when he’s high.

Yes, you should be checking ketones every day while he is this high.

It looks like you shot 1.25u this morning and 1.5u this evening. I would continue with the 1.5u for three more cycles unless he drops below 50 which I doubt he will do. After a total of four cycles at 1.5u, we should reassess and talk about the next plan. I’m almost certain you will need to increase to 2u at that time but let’s see.

A cycle is 12-hours so there is an AM and a PM cycle every day.

You can either get a ketone meter or use urine test strips. Both are good it's just a matter of your preference.

Patricia….you don’t need to link the spreadsheet in a post. It’s already in your signature.

I’d suggest increasing to 2u every 12 hours for four cycles. It’s best if you can try and always get a +2 both cycles. During the day, any other midcycle test in addition to the +2 is helpful. In the evenings, a +2 and a before bed also works.
