Hello,
My name is Heather and my diabetic kitties name is Bug. He was initially diagnosed with diabetes in March 2021 and was well regulated on 2u BID Glargine with blood glucose numbers consistently around 100. I test regularly with an AlphaTRAK and in January 2021 he went into remission. Bug is 15 years old with renal failure so tends to use the potty and drink water quite often. I give him subq fluids at home and he’s on a low carb, high protein, low phosphorus wet food.
I tested every month while in remission to make sure his numbers stayed in the normal range which they did until early December 2022. He tested at 474 and was showing poor clinical signs- excessive thirst and ruination, unsteady gate, glossy eyes, lethargy. We moved states and thus changed vets during the time he was in remission so we scheduled a visit with the new vet to confirm his diabetes had returned and to get a new rx for glargine. They recommended we start him on 2u since he did so well with that dosage previously. They did blood work to check kidney values and his BUN (blood urea nitrogen) was quite high so I’ve increased subq fluids to 150mL daily.
Insulin therapy was started on 12/13/2022 but Bug’s numbers have consistently been in the 400-500 range with the exception of a couple pre-shot numbers in the high 300s. I’ve increased his dosage about every 3 days (or 6 cycles) with no decrease in blood glucose thus far. He is now on 4.5u BID. Ive tested his urine for ketones a couple of times which have been negative. I realize his kidney failure is most likely not helping with diabetes regulation, but shouldn’t I be seeing his numbers decrease as I increase his dosage?
I plan to create a spreadsheet soon (laptop died and new one is on the way) to show what his curves have looked like and to give you more information, but to give you an idea here are the results of his last two curves.

I’m looking for advice with how I should proceed. Do I continue to increase dosage until I see numbers start to drop? At what point do we consider insulin resistance due to another potentially underlying health issue (besides the renal failure)? Any advice would be greatly appreciated. Please also let me know if I need to clarify anything. Thank you! I’m very appreciative of the vast amount of information I’ve received from the website and forums so far!
