My name is Yolanda and my cat is Tiger (16 y/o, neutered male).


A bit of history prior to diagnosis.
I have other cats, so ai wasn't sure who the culprit was for a while but had been finding urine behind the litter boxes for a couple months. I finally caught Tiger peeing on the floor after staking out the litter box room. I also noticed that he had become so hungry. He always liked to eat so at first it was nothing abnormal until it was his usual begging for snacks but counter surfing and trying to take food off our plates, eating like he hadn't eaten in days. He's always been at a healthy weight but ai noticed he began to look thinner, despite eating normally. (Had been on kibble with some canned food a few times a week). I also began to notice that he was hanging out near the water fountain and was drinking more than usual, the amount of urine output I would find on the floor or litter box was wayy more than I'd ever seen any of my cats. I started to suspect it could be something to do with his kidneys or diabetes.


Our vet was pretty busy so we had an appointment booked to have him checked out. I noticed he seemed pretty lethargic one day and it worried me so thankfully the vet managed to squeeze us in about a week before the appointment.

He did a urine and blood test. His initial thought wad that it was his thyroid because he also seemed anemic. His gums were quite pale.

Unfortunately this was over the long weekend, and after a super busy week for them we didn't get the results until about a week after. He called me to let me know that he had diabetes and wanted to have him brought in to do a glucose curve + start him on insulin. The testing and vet visit was already expensive and I'm not in the best place at the moment so I suggested I could do the glucose curve at home. Apart from it financially being better for me, I knew it would be better for Tiger. He gets so stressed out at the vet. When they pulled blood, I could hear him growling and screaming down the hall from the exam room I was in. When they brought him back to me he was panting and laying on the table looking so lethargic and dull. Even the vet looked super worried when he saw him like that. He made me stay longer to give him subq fluids and let him eat until he perked up before going home.

Vet told me he would need to confirm with the clinic manager about letting me to the glucose curve and that took another few days to finally hear back.

They said I would need to sign a waiver that I refused to do testing at the clinic and releasing them from any liability. I understand that and was fine with it but I wanted to confirm that this didn't mean I was looking to go at this alone. I wanted to do the curve, send him the info and go from there so he could watch over and advise as needed.

Since getting a diagnosis 07/06, I went out to buy him a low carb wet food and have been feeding only that. I needed to wait to get paid again to be able to buy the insulin and supplies to test. When we picked up the insulin a few days ago (Prozinc), the vet wanted to start him on 2 units.

I have everything now and did my first glucose test using the Bravo meter.
He tested 6.7 mmol/L before feeding. His ear bleed easily and I had no problem getting a reading but wasn't sure what the number meant so I went on the FB sister group to this form to double check before giving insulin.

To my surprise, I was told that was a pretty close to normal number and that sometimes diet alone was enough to help them regulate. I was advised to hold off on insulin and I will be retesting his glucose every couple hours to see if that was an accurate reading or if anything changes throughout the day.

I guess I'm confused now and a bit frustrated. If I can regulate him on diet alone, that's wonderful but I'm out $150 with a full vial of insulin and the dose I was told to start at could've been dangerous since his numbers are not high. At the vet, he was tested and it read 11.6. I don't know if that was due to stress or if the number came down due to the diet change I implemented.

I feel as though I should maybe look for a new vet. He is honestly very nice but perhaps I'd feel more comfortable with someone who is more collaborative, if that's the right word. It would be nice to feel a bit more support and advice rather than just being given instructions on dose and how to give insulin but not really be advised of what to look for/things to be aware of/ what numbers mean and what I should be aiming for.


I started a spreadsheet for Tiger. I'll update after I do all his testing today. I guess what I'm wondering is, if the diet alone is enough to regulate him how long does it take until I see changes in him (ie. The extreme hunger to normalize, the inappropriate urination outside the litterbox to stop)?

Dealing with the urine outside the litter box is the hardest part. I'm constantly cleaning and even though I've put down pee pads he has the habit of pushing them out of the way to go on the floor. It seeps through the tile cracks and goes to the hall way. It's so stressful and I live in a rental so I'm so worried about the smell. It's really draining me and I'm just hoping it'll get better.

He doesn't have arthritis so it's not a matter of him feeling too painful to go into the box. He jumps all over the place including his gigantic cat tower that basically goes to the ceiling, and places I'd rather he not jump like the top of the fridge to look for bread bags ( learned the hard way to not keep them there anymore). He always has used the litter box, this only started when I began to notice the other symptoms. Has anyone used the litter attractant with a diabetic cat and if so did it encourage then to go in the box?


If his sugars remain stable today, should I go to the vet for anymore testing? I need to be conservative about what tests I can do but is there anything that may be beneficial to help us understand if we're on the right track/ if I should even use insulin?

Thank you, just did that. What does TR or slgs mean for dosing?
The 6.7 mmol/L convert to 120 mg/dc (units used in USA). This BG is really not diabetic.
Did the vet give you the values(s) he used to determine that the Tiger was diabetic?
Did you give any insulin? Even 1 unit may be too much.


I have held off from starting the insulin based off the glucose reading I got today. The glucose reading at the vet at the beginning of the month was 11.6. We got a diagnosis of diabetes after getting results back from the lab for the bloodwork + urinalysis.

Since getting a confirmation of the diabetes, I have only been feeding low carb wet foods. I'm not sure if diet alone could've caused a change in his sugars within a couple weeks. I had to wait a couple weeks to get paid to be able to buy the insulin and glucometer, lancets, syringes but wanted to at least start the diet change in that time.

Today was going to be the first day starting on insulin. Now I don't know if I should just stick treating with a low carb diet or if there could be something I'm doing wrong that is giving me a wrong reading.

Until you hear from her she will probably be on tonight I wouldn't give any insulin until you hear from her but keep testing. How long ago did you stop the kibble?

Melissa ,Yolanda has not started prozinc yet , switched to an all lc wet , vet wanted her to give 2 units of prozinc good thing she didn't give any
Can you look at her SS. She stopped kibble about 3 weeks ago

I didn't want to take a risk without understanding what the numbers meant and what range normal was. Glad I asked on the Facebook group. I'll continue to test every couple hours today. I stopped kibble about 3 weeks ago. He managed to steal a couple pieces of kibble once from the other cat (escaped the bathroom and went straight for his brothers bowl) but apart from that has had zero kibble since.

I'm glad you checked ,like Larry posted even 1 unit would have been too much to start. Melissa usually comes on around 9 PM eastern time
 
I too would skip insulin but keep testing. Also be sure to check ketones as well.

Yes, that has not improved at all. Vet initially told me on the phone he wanted to start him on antibiotics since a lot of diabetic cats gave utis or some kind of infection. When we picked up the prozinc there were no antibiotics or notes about it for the receptionist to go over with us. He wasn't in that day so we have to call when he is in office to ask about that. I don't know if it was a precaution or if he saw reason to want to do antibiotics based off his labs. Wish I was given some more clarity. I don't think he'll be in again till the week so I'll have to wait a couple days.

I'm don't know much about uti's or any type of infections , but there has to be a reason why Tiger is still peeing outside the litter box especially with the great numbers you have gotten so far on your SS

Just some info Tiger has not started prozinc, vet wanted to start with 2 units, Yolanda took BG this morning and was told to not give insulin by a Facebook group Glad she didn't .Has been on a lc wet diet, last time had kibbles was about 3 weeks ago

Waving from Ontario. Welcome.

Some "if's" for you.
If Tiger does need to take insulin, you will need to have a lot of test strips on hand because you cannot get Bravo strips at a drug store.
If Tiger has a hypo event, you will go through more strips than you can imagine.
If Tiger does need to take insulin, ask the vet to switch to Lantus when you finished the bottle of Prozinc. Lantus is much cheaper in Canada, especially if you buy it from a drug store instead of the vet.

Unfortunately this was over the long weekend, and after a super busy week for them we didn't get the results until about a week after. He called me to let me know that he had diabetes and wanted to have him brought in to do a glucose curve + start him on insulin. The testing and vet visit was already expensive and I'm not in the best place at the moment so I suggested I could do the glucose curve at home. Apart from it financially being better for me, I knew it would be better for Tiger. He gets so stressed out at the vet. When they pulled blood, I could hear him growling and screaming down the hall from the exam room I was in. When they brought him back to me he was panting and laying on the table looking so lethargic and dull. Even the vet looked super worried when he saw him like that. He made me stay longer to give him subq fluids and let him eat until he perked up before going home.

Vet told me he would need to confirm with the clinic manager about letting me to the glucose curve and that took another few days to finally hear back.

They said I would need to sign a waiver that I refused to do testing at the clinic and releasing them from any liability. I understand that and was fine with it but I wanted to confirm that this didn't mean I was looking to go at this alone. I wanted to do the curve, send him the info and go from there so he could watch over and advise as needed.

Since getting a diagnosis 07/06, I went out to buy him a low carb wet food and have been feeding only that. I needed to wait to get paid again to be able to buy the insulin and supplies to test. When we picked up the insulin a few days ago (Prozinc), the vet wanted to start him on 2 units.

I have everything now and did my first glucose test using the Bravo meter.
He tested 6.7 mmol/L before feeding. His ear bleed easily and I had no problem getting a reading but wasn't sure what the number meant so I went on the FB sister group to this form to double check before giving insulin.

To my surprise, I was told that was a pretty close to normal number and that sometimes diet alone was enough to help them regulate. I was advised to hold off on insulin and I will be retesting his glucose every couple hours to see if that was an accurate reading or if anything changes throughout the day.

I guess I'm confused now and a bit frustrated. If I can regulate him on diet alone, that's wonderful but I'm out $150 with a full vial of insulin and the dose I was told to start at could've been dangerous since his numbers are not high. At the vet, he was tested and it read 11.6. I don't know if that was due to stress or if the number came down due to the diet change I implemented.

I feel as though I should maybe look for a new vet. He is honestly very nice but perhaps I'd feel more comfortable with someone who is more collaborative, if that's the right word. It would be nice to feel a bit more support and advice rather than just being given instructions on dose and how to give insulin but not really be advised of what to look for/things to be aware of/ what numbers mean and what I should be aiming for.


I started a spreadsheet for Tiger. I'll update after I do all his testing today. I guess what I'm wondering is, if the diet alone is enough to regulate him how long does it take until I see changes in him (ie. The extreme hunger to normalize, the inappropriate urination outside the litterbox to stop)?

Dealing with the urine outside the litter box is the hardest part. I'm constantly cleaning and even though I've put down pee pads he has the habit of pushing them out of the way to go on the floor. It seeps through the tile cracks and goes to the hall way. It's so stressful and I live in a rental so I'm so worried about the smell. It's really draining me and I'm just hoping it'll get better.

He doesn't have arthritis so it's not a matter of him feeling too painful to go into the box. He jumps all over the place including his gigantic cat tower that basically goes to the ceiling, and places I'd rather he not jump like the top of the fridge to look for bread bags ( learned the hard way to not keep them there anymore). He always has used the litter box, this only started when I began to notice the other symptoms. Has anyone used the litter attractant with a diabetic cat and if so did it encourage then to go in the box?


If his sugars remain stable today, should I go to the vet for anymore testing? I need to be conservative about what tests I can do but is there anything that may be beneficial to help us understand if we're on the right track/ if I should even use insulin?


Long post and a lot of information. So sorry for that, I'm just feeling like my head is spinning. I want to make sure I'm doing whatever is best for him and not putting him in a potentially dangerous situation.

Was a urinalysis performed and can you share the results of thos? With inappropriate elimination going on, they must have done a urinalysis. The labs are showing likely impaired kidney function. Didn’t your vet even mention this. I have not been able to read through every response your post yet, but I carefully read through this information you shared above.
 
Also, I used to have a problem cat who didn’t always use the litter box. I found that the Dr. Elsey’s Cat Attract litter really got him using the box again. I hate clay litter and don’t ise it anymore, but of I had a problem litter box user again, I would definitely buy this litter again until I got things sorted out. Just a thought.

He did mention slight kidney impairment but that it could just be from the diabetes not being controlled. At that point he mentioned wanting to do a round of antibiotics. When we went to pick up the prescription we were only given the prozinc and the receptionist had no notes about the antibiotics. I'll have to wait a few days till he's back in to ask about that.
 
By the way, you did the right thing in reading and educating yourself about FD and in not starting insulin injections yet. You may have saved your vat’s life. As others probably have already said, Keep up the testing and charting it on your spreadsheet for a bit so we can see if he needs insulin at all.

Okay yes. I see it now. They should have sent it out to culture it at a lab. As it is there’s blood from irritation and a moderate amount of rods (rod-shaped bacteria) in his urine, so I would say a round of a broad spectrum antibiotic is definitely warranted. I bet someone at the vet just forgot and dropped the ball on that since they were focusing on diabetes. I would call Monday to make sure I got the antibiotics.
 
And I do agree that if he really does have diabetes that can skew some of the labs (not all) bit another thing that skews labs is having a urinary tract infection and you may see an improvement in some labs after treatment.
I'll definitely ask the vet about the antibiotics as soon as he's back at the clinic. The insulin has never been used..... I'm hoping if he doesn't need it that they'll take it back. It cost $150 and like I also explained to the vet, I'm tight right now. I'd be super miffed if I'm out $150 for something I won't even need. I'll see how the testing goes over the next few days and I guess if he really doesn't need it hope they'll take it back. I feel like I wasn't given much information or guidance. I'm very grateful to the fb group as well as this forum. I've learned a ton, relatively easily in a short amount of time. I'm extremely grateful I did not follow through just giving the prozinc at 2 units like advised. I understand about the peeing outside the box! And don’t you just love it on tile when the grout channels the urine all over the place. Been there, done that.

It's awful and sadly my only option as I have dogs (one who thinks cat litter is a delicacy) and everything else is hardwood. I have woken up far too many times and stepped in a puddle of urine that has seeped into the hallway as I make my way to the bathroom. Pee pads have at least been able to control that from happening lately
 
If he doesn’t end up needing that insulin and you are unable to return it for a refund, we have a “supply closet” here where people sell (or give away) insulin and other supplies. You may have someone willing to purchase the ProZinc and be able to recoup some of your expenditure for that insulin. You would just need to be able to ship it with an ice pack.
