Hey all Musette flies home to me tomorrow, and I could really use your input on where to start her dose. I don't have this weeks numbers yet from her foster dad, but I hear she is a really bouncy girl. Her foster has her on .75u and doesn't think she is rebounding, I'm not so sure I agree, but then again he works long hours so hasn't really curved her yet.

Tomorrow night I know she is going to be high as she is only being given a token dose of insulin before she flies tomorrow. If you could look at the numbers that I have so far...do you think it would be reasonable to start her out on .5u? Hold for a few days and see what that gets me?

Musette has already survived DKA once back in Mar, so those 300-400s scare me. She is already on a proper diet and will stay that way of course here. She was tested yesterday, no ketones present and I would like to keep it that way.

Mel,

Congratulations!

I'm sure you'll be testing as soon as she gets to you. If she were mine, I might spend a cycle or two collecting data before shooting, unless there are some more recent numbers that just aren't on the spreadsheet yet. Based on what is there, I'm not even sure she needs 0.5u. She hit 40s on that dose, though it did come right after a 1u shot so there could have been some shed action behind it. Then she missed a shot, but hit low 50s again on only the 2nd shot of 0.5u. At this point, I feel uncomfortable being more specific about dose without recent data.

Obviously if she starts to show ketones you wouldn't want to delay shooting, so it's a balancing act. Will you be posting her numbers here when she arrives?

Things can change quickly when they are at that stage of the game, though, so this week's numbers might not look anything like those. When was Musette diagnosed?

There are more numbers I just don't have them yet, they will come with her tomorrow. But when I talked to her foster yesterday she is still bouncing all over the place. And he was concerned enough to take her to the vet's.

I have all her vet records, and she was dxed FD & DKA in the same breath in Mar, survived, then in June she was surrendered for PTS, DCIN stepped in had her pulled and is in foster care with Don & Chubby. He is testing her so I will have more numbers soon. But his vet said up her to 1u, but seeing what she did on that I'm not comfy with it. If she is bouncing on .75 that says to me drop back, and start over.

She has had a pretty rough life so far, I just want to give her every chance to be healthy, happy and loved, probably for the first time in her life..Her previous owner wouldn't give her insulin on a regular bases. declawed her and threw her outside. Sometimes I hate people...sigh

Congrats on the new arrival!! I suspect there will be a lot of people here who will be behind your wanting to give Musette the best chance for happiness possible!

I agree that she's bouncing and that 1.0u is too much. Other than that, once you have this week's data in your hands it will be much easier for you and for us to help you sort out dose. If Musette's numbers are high and she's throwing ketones when you get her home, post and we'll help you figure it out.

I'm sure you will be seeing a lot of me here while I get this little one figured out. She is a pretty little thing, as far as I can tell she is a purebred Himalayan, but only tips the scales at 6lbs. I don't remember when I last had a cat that small, and that is extremely small for a himmy. So she is either way underweight, or she was stunted in the very beginning. It still just blows my mind that her owner could have her for 11 years, nurse her through DKA and then want her destroyed 3 months later. Especially since if she is as I suspect a purebred, they paid a pretty penny for her, as she is a lavendar point, a hard color to get in a himmy. Oh well their loss is my gain!

congratulations! i think you're definitely on the right track. a reduction does seem to make the most sense. at 6 pounds, she a tiny one! hope she settles in to her new home quickly.

looking forward to seeing whatever data you're able to collect over the next few days...

Here's my new baby at her foster daddy's house...shhh no laughing cuz she is nekked. Apparently I can only adopt nekkid FDs... :lol: I swear they had fur when I adopt them then people go and steal their clothes.

She's absolutely gorgeous (no laughing here)! She is the luckiest girl there is! How many insulin dependent cats are you caring for? (Sometime you'll need to give me hints on how you manage so many cats - I have 10) :-D .

insulin dependent.....none! I broke my last one in 2 weeks...lol. Lucky for me DCIN didn't make me return him to get a replacement model.

I have a t-shirt that I love..it simple says "Rescue is my favorite breed!"

Too bad more humans can't learn a thing or two from animals. The same drooler that was beaten and starved...took a bullet in the shoulder a couple months ago trying to protect our neighbor's property from a home invasion, these same neighbor's allow their kids to tease and throw rocks at him. Yet he was willing to lay down his life for them. They did at least pay his vet bill and gave him 3 lbs of bacon. And even after all that he is a big sweet goof ball that loves his kitties, he thinks they are small clever puppies and they think he is a large, clumsy, stupid cat. When he was recovering from being shot, they took turns laying with him, grooming him and bringing him their catnip toys.

I do urge caution with her ketones and pH. I have the advantage of having seen her vet records, and when DCIN was having her boarded before rescue, Musette was discharged with trace ketones in her urine and a pH of 6. Her ketones have since tested clear, but I do wonder about her pH and sent you a link in an email, Mel, about the regeants stix that test 10 parameters, including pH and ketones. I know that tender care and a good diet will bring her around, and make dosing decisions easier, but I do worry about ketones and pH.

I just want to know why you have to adopt nekked kitties?! Can't you adopt any with fur coats? OK so it's summer and warm out and they don't need fur coats right now, but still.....what gives????

God bless you, Mel, for taking in such a beautiful creature. Thank you for giving her a wonderful, loving, caring forever home. She deserves that more than anything. How could anyone want to PTS a beautiful little girl like her? It's absurd! I, too, don't like humans (except for you guys, of course.. LOVE you guys *HUGS*.)

I bet she feels much better without her coat right now on such hot days. She's such a little love. :)

I just did a quick search on urinary pH in cats. I've seen ranges for normal as low as 5.7 vs. 6.3 (the latter being from the Merck Vet Manual). Depending on what reference range was being used for the test, a pH of 6.0 may be normal. Once Musette is settled in, re-checking pH and urine specific gravity (USG) sounds like a good idea.

Safe travels today (Omaha, right?)!! The story of your kids is just amazing and heartbreaking all in one. Love the image of the kitties nursing your dog back to good health... complete with catnip toys!! Made my weekend!

Musette is just gorgeous. Can't wait to follow her progress!!

Thanks for that input Sienne. I am sure that Mel will consult with her own vet and with her own reference materials on the acidotic level of urinary pH.

The probability that Musette's pH may be too low was presented to me by Musette's vet in Virginia. This site says that a cat's urinary pH is normally slightly acidic and that a range of 6-7 would be normal.

However, Understanding Diabetic Ketoacidosis from the 30th World Congress of the World Small Animal Veterinary Association states:


I haven't a clue what the relationship between blood pH and urinary pH is.

Feline Diabetic Ketoacidosis, a scholarship research paper by Christi Benigni in the Fall of 2008 states:

The normal pH for a feline is 7.4 and is necessary for the normal function of many enzyme systems. When pH drops below this value the cat is referred to as being acidotic.

Mr. Benigni does not say whether this is blood pH or urinary pH.

This is the Internet and we all know that you never know what information to accept as gospel. At least we agree, though, that it is important to keep an eye on the Divine Miss M's pH.
