Neuropathy - BAD

My cat Shaklespeare was diagnosed 7 months ago (May) His number was only 126 so the vet put him on a speacial diet of Hill's and since then he has gone downhill.
We started Humulin insulin in September because his numbers were around 300.
So I started 3 units 2x a day.
October and Novermber were terrible and the Neuropathy got so bad he could barely walk so I start the Methly B-12, 5 pills a day which seemed to help.
One month ago I changed vets (long story) and the new vet put him on Lantus - 3 units 2x a day.
For the first 5 days he improved so much, he was standing and walking better, but after a week or so his neuropathy once again is really bad, he cant jump and barely walk. I reduced his Lantus to 2.5 untis 2 times a day and did a curve Sunday
Here are his numbers:

Insulin given at 5:30am
8am 275
10am 246
12pm 183
2pm137
3pm 147
4pm 167
6pm 281

They arent bad numbers from what I have been told but today he can barely walk
Any advise would be great
 
I know very little.... but did you miss one important test?-- the one before shot.

Did you switch to "wet food only"? It helps lower the BG a lot. When my cat's just diagnosed, my vet asked me to feed Hill's m/d but I learned it is still too high in carbs from this board. I am feeding my cat Mimi Wellness, Fancy Feast classic, Merrick, and some other low carbs canned food.

Mimi was once not able to jump and walked on hocks, sit down every few steps.... He didn't improve much until one month later, very little by little. So give it sometimes.
 
I think you need to get more data before you can determine what is going on. I would suggest you start doing daily testing, getting pre shots every time and mid cycle number every day. Start a spreadsheet and get those numbers on it so you can see patterns.

Wet lo carb food can make a huge difference. When we switched from dry to wet, Oliver went down 100 points overnight. If I were you, I would switch over to all wet.

The B12 helps with neuropathy but it has to be combined with consistent lower numbers. Your curve does look pretty good, but I would like to see a weeks worth of numbers.

There are other issues that can cause the movement problems like a clot or stroke, but I have no experience with them.

you need to use the correct form of B12 for neuropathy and it's methylcobalamin.

why -

1) doesn't contain any sugars or fruit flavorings, like other brands
2) easy to administer - empty one capsule daily and mix into wet food
3) there is more than enough of a dose, that if the cat doesn't eat it all, no big deal - typically recommended daily dose is 3-5 mg. this contains 5 mg
4) in general - if you have other cats and they eat it, it won't hurt them

Thank you both for your input. Shakespeare has been on only wet food for a few weeks now. Fancy Feast Classic Beef only, which from what I read has the lowest carbs. As a treat after the insulin I will give him a bit of turkey breast. I have been testing his glucose every morning and night preshot and it isalways in the mid to upper 200's
I am slowly lowering his dose of insulin and I am seeing a neurologist tomorrow.
Hopefully they will be able to give me some answers,

I think that the treatment at the start was flawed (3 unitsto start) and continued when you were told to give 3 units of lantus. Given those relatively high doses and relatively low numbers plus worsening neuropathy, I would seriously question the initial diagnosis and whether it is in fact neuropathy. I hope the specialist can sort it all out
