negative urine??? help

My cat was DX with diabetes 1 mo ago.. put on Protomine 5 units bid. After 7 days his sugar was still very high so the vet
changed it to 6 bid. That did the trick... no more weak legs, lethargic, peeing on floor etc! He is currently 27 lbs, down for 37 a year and half ago, (large cat, Maine Coon.) He is no longer eating Royal Canin Satiety, but 3 days ago was switched to Young again, a no carb diet.. last nite for no reason he peed on the floor, and when I tested the urine he tested negative
for ketones and sugar.... none! The vet told me to monitor the urine closely for the next 3 days for a positive reading, as he feels he may have ceased being diabetic. My problem is I am lving for vac on Sat, and will not b e able to monitor him closely .. I have a petsitter coming 2x day for feeding and to give him the shots....
What do I do.... lower his dosage, stop it for a week???? HELP!!

No sugar in his urine means his blood glucose is under 270 (the renal threshold) at least that day - it could be high again later or tomorrow..

Can you quickly go out an pick up a blood sugar monitor and test him at home before you go? That way we will know how low he actually is and what dose you could give.

Heres a shopping list and testing tips below

Getting started shopping list
1. Meter ie Walmart Relion Confirm or Micro.
2. Matching strips
3. Lancets - little sticks to poke the ear to get blood . new members usually start with a larger gauge lancet such as 28g or 29g until the ear learns to bleed. Optional - lancing tool.
4. Cotton balls to stem the blood
5. Neosporin or Polysporin ointment with pain relief to heal the wound
6. Mini flashlight (optional) - useful to help see the ear veins in dark cats, and to press against
7. Sharps container - to dispose of waste syringes and lancets.
8. Treats for the cat - like freeze dried chicken
9. Karo syrup/corn syrup or honey if you dont have it at home - for hypo emergencies to bring blood sugar up fast
10. A couple of cans of fancy feast gravy lovers or other high carb gravy food- for hypo emergencies to bring blood sugar up fast

Re:REMISSION

After taking insulin for 4 wks.... Cosmo is now in remission. He was taken off insulin for 10 days and is still
showing as neg for sugar....
 
If you mean negative urine tests, his glucose may be below the renal threshold of about 240 mg/dL, but that still can be high enough to cause damage over the long term. Did lab work get done showing a good fructosamine level, with blood glucose in normal ranges too?
 
I agree with BJ - I would take him to the vet to run a fructosamine curve to be sure he is in normal range. Normal range is 50-130 but negative urine sugar could still mean he goes up as high as 270 which is too high and will cause him issues long term!!.

Or you could buy a human blood meter for $20 and test his blood yourself to be sure.

Tips to stay OTJ

We say a cat is in remission if the cat can maintain BG levels for 14 days between 40-120 with most of that spent under 100. However, please keep in mind that once a diabetic always a diabetic.

1. Never feed dry - not even treats. If you change wet food types, be 100% sure the new food is also low carb and same low carb % as your current food. Some cats are very carb sensitive and an increase from 3-6% to 8-10% can spike the BG’s. Don’t feed if you aren’t sure!
2. Weigh every 2 weeks to 1 month to watch for weight changes. Too much of a weight gain can cause loss of remission.
3. Measure blood once a week, indefinitely. You want to catch a relapse quickly.
4. No steroids or oral meds with sugar - remind your vet whenever giving you any medication. Always double check.
5. Monitor food intake, peeing and drinking. If increasing, a sign of losing remission.
6. Regular vet checks for infection such as dental , ear or UTI. And get them treated quick!

If he does fall out of remission you need to be more aggressive and resolve issues/ back on insulin as soon as possible as the window for a second remission is tight if any.
