Need help, trying to remember how to do this

Hi everyone, my kittie’s been in remission for a year but it looks like his FD is back.
His BG numbers have been WAY up the last 10 days, in the high 300’s, (even got a 407). We thought it was the result of Budesonide, (the vet thinks he may have IBD, his diarrhea wasn’t responding to Fortiflora.) He was only on it for 6 days but the start of his high #’s coincided with starting the steroid.

I took him off the steroid 3 days ago but his numbers haven’t gone down much-in the 340’s at the lowest.
Vet says he’ll have to go back on insulin if his numbers don’t go closer to normal soon.
I am so embarrassed to say this, but I’m having such a hard time remembering how I did it :nailbiting:
I’ve been reading the stickies and posts, but can’t remember when I give the injection:
-do I feed after the AMPT, then OK to shoot because he’s eaten a meal and it’s safe to do so?
- or do I go ahead and shoot after I test, and then feed him, hoping he’ll eat?
I feel so dumb for not remembering this, I really need your advice and thoughts.
I don’t want to make a mistake that causes his blood sugar to plummet dangerously low.

Steroids can increase the BG. IBD could also, might consider raw food, I've had lots of luck. Any kind of infection could also cause an increase. Get some blood work if you haven't already.

Test-feed-shoot. I wait about 10 minutes between food and shoot, just to make sure he eats it and doesn't throw up.

Florastor has been recommended to me instead of Fortiflora as it has a larger number of good bacteria. You can give about 1/4 capsule twice daily for maintenance and up to 1/2 capsule twice daily if Scout has diarrhea.

I also give SEB or Slippery Elm Bark. The website I gave you has instructions on how to prepare and give SEB. That helps with calming the gut and also other things like nausea.

I've also heard Budesonide doesn't affect BGs too much but ECID.
 
It's also not necessary for Scout to eat a full meal during shot time but I would still wait for him to finish and watch for vomiting since he's having possible IBD issues. But as long as Scout eats during onset which can be 2-3 hours later, having Scout eat his entire shot time meal isn't required.
 
Ok Crista, so it would be OK for me to can while he’s 2/3 of the way through, almost to the end of a meal ?
 
Sorry do you mean if it's okay to shoot while he's 2/3 done his meal? Personally, his numbers are pretty high so I would feel comfortable shooting. A lot of people give insulin while kitty is still eating.

What dose are you thinking of giving?
 
I feed him a LID, Royal Canin venison, and it has been pretty effective at controlling his IBD symptoms & his chronic pancreatitis. This summer has just been tough for him with 3 pancr. flares.

Yes-i meant is it okay to shoot when he’s 2/3 done -before he’s finished .
Re: dosage, I will check with his dr. but I wouldn’t want to start with anything higher than .5 or 1.0
 
Also just wondering, is your SS complete? Did you stop insulin on 11/27/18?

I would think so too. But I'm not comfortable giving dosing advice. I'll tag some who may be able to help: @Wendy&Neko @Sienne and Gabby (GA) @Chris & China (GA)

Did you follow a method here in the past? TR or SLGS? Or were you following your vet's recommendations?

When you do shoot insulin, please make sure you're able to test throughout the day the first couple of days. And also get some tests at night, even just a before bed test. Cats often go low at night.

I personally, and some other members, don't believe food has much to do with pancreatitis in cats. The important thing with pancreatitis is staying on top of issues such as nausea, hydration and making sure they eat. I see you are on top of the nausea part with cerenia and ondansetron.

How is Scout's appetite?

Hopefully members with experience with IBD will chime in.
 
The SS is up to date, he stopped Lantus in August 2018. 2nd remission.

Right now his appetite is very good.
Using the Alpha Trak meter, method is SLGS.
The LID diet has helped his pancreatitis greatly, it helped him turn the corner with the pancreatitis 7 years ago-along with the supportive meds. He also gets fluids, Cyproheptadine, Famotidine and Metoclopramide depending on flare symptoms. The IDEXX paper on pancreatitis has been what’s helped me and guided me in making sure he gets support he needs.
 
Using Lantus or Levemir, you Test/Feed/Shoot....all in about 5-10 minutes.

Test to make sure he's high enough for insulin, Feed to make sure he's at least willing to eat, and Shoot.

Since the shot doesn't usually start to "kick in" for 2-3 hours, as long as he's willing to eat, you have some time for him to eat his meal.

I'd get him back on insulin....I'd start at .5

Can you add which insulin you're using to your signature? There are a few differences between Lantus and Levemir

Good Morning! When you can, please right click on the tab (at the bottom of your SS) and it will allow you to change the title from Sheet 1 to '2018-19' . There are a lot of tabs there now, so when we open the SS link we see 2018 and we're thinking it's an old SS.

For your signature, you will want to add 'type of meter', 'type of protocol', 'type of food', 'your name' .. There are limited characters so you might want to look at some other signatures to see how we've abbreviated, etc.

You're off to an excellent start! So sorry to see Scout has come out of remission - let's hope he goes back to the Falls soon!
