Need Help blood sugar 73, 99 and 119

So my cat is newly digagose with diabites and today after having him on 3 units of Latus per the vet I did his cruve today his blood sugar was very low 73 this morning at 10 am and then 99 at 12:30 PM i called the vet and told them and no one has gotten back to me I did his last check at 5 PM and his blood sugar is 119 I am very concerned about giving him his inslutin again at 6:30 PM. I give him his shots at 6:30 AM and then again at 6:30 PM according to the vet. Any help
 
Couple of quick questions:
1) What are you feeding your kitty
2) did the vet start you off at 3u twice per day?

btw, the first 2 numbers are not bad at ALL.. it is the 119 BGL that concerns me when his next shot is due in only 90 minutes after that test.

I would skip the shot. For newly diagnosed diabetics, we recommend not shooting Lantus if under 150. A lot of people will not shoot insulin if under 200, if the cat is newly diagnosed.

The 73 and 99 and 199 are nice healing pancreas numbers, by the way.

Normally, a cat is started on 1 unit of Lantus or less. Did the vet tell you to shoot 3 units right off the bat?
 
the vet started him at 1 unit about a month ago his blood sugar was in te 400 and 300 then we went up to 2 for about a week and then I did another curve and it was in 200 so then vet bumpped him up to 3 units and then i did another curve today wich is where these numbers came from. I was thinking about skipping the shot tonight and then checking him tomorrow morning an maybe reducing it to 2 or 1 unit until I can talk to the vet depending on his numbers. thoughts
 
Thanks for the info.

We normally make dose adjustments in 0.25 units, and only after seeing what the mid-cycle numbers are and holding the same dose (unless the numbers are really low) for at least 3 days.
Vets are just used to doing 1.00 unit dose changes.

You can stall without feeding and see if he comes up enough for you to give the shot, but that would put your shot schedule off by however many hours that is. Meaning, you would need to shoot your next shot (and others following) 12 hours after you shoot tonight, if you do.

I would skip the shot and reduce as Dyana says. However I think you might need to do a little more testing to keep him safe - we dont want any scary drops at night. So can I ask a few things

1. Test before every shot, but also test midcycle when you can ( like 5-6 hours after his morning shot) and once before bed (which is a few hours hopefully after his evening shot)

Also what food brand/type are you feeding? wet ? dry?

I do keep track of his progress I try and take a pre shot test in the morning when I can and I do one in the evening as I have more time. I use the Alphatrak 2 and the reports to help keep track of him. I skipped the shot last night as I didn't feel comfortable giving him his shot I checked him this morning and his blood sugar was 283 so I gave him 2 units this morning then I tested him again this evening before his evening shot and it was 223 so I gave him another 2 units. I would test him more often but he does not tolorate the ear pricks well and he moves a lot so it takes me a good 15 to 20 minutes to get a blood sample from him. It is stressful for him and I don't have anyone to help me perform the tests. I will consult with the vet tomorrow regarding his curve from yesterday my feeling is that he might have to be put on 2.25 units or 2.50 units to get him to normal range, however since I don't have any surgins and I am using the pen that only has whole units I have to give him 2 for now.
 
Many of us use the pens but we dont use the needle tips since the dosing from those isnt that accurate. I mean its ok if you are a human and its out by say 0.5 unit either way but to a cat thats a big deal. So we buy syringes (3/10cc insulin syringes with 1/2 unit markings) to withdraw the insulin. There is a rubber seal on the pen and you just insert the syringe through it. And its more accurate and allows smaller doses. (ie Relion 3/10cc 30 & 31 gauge short, BD Ultra fine 3/10cc short, Terumo Thinpro Insulin Syringe 31G 3/10cc.)

Can I suggest you measure him a few times mid cycle before you change dose? You want to base dose changes on his low point 5-7 hours after his shot, not his high preshot levels. For example say he drops to 40 mid cycle when the Lantus really kicks in and then shoots up to 280 by the next shot? You certainly wouldnt want to increase him then!

I would love to be able to test him mid day however I have a full time job and I work 45 minutes away so I am pretty limited to when I can test him. I will try at night if I can get him. But I reduced because of the curve yesterday and I was doing what my vet has instructed me to do which is give him full units for now. I truely appricate all the help you all have been great. I will keep you posted.
 
Well maybe set the alarm for during the night or get a before bed test? I was stunned when I found Bailey dropping to 35 at night having been 275 preshot. I always do a before bed test now just to check.
 
So I spoke with the vet today and she instructed me to bump him down to 2 units and then check him again this weekend. Thank you all so much for all the help I really appricate it!
