? Need dosing advice ASAP!

Well, Squallie has managed to completely confound me tonight. He is 224 at PMPS and there's no way I can give him the dose he's been getting lately, with BG that low. I now have no idea how much to give him tonight. Can anybody take a quick look at his SS and maybe give me some guidance? Thanks!

ETA: I've been so freaked out by his recent high BG that I've been testing him like a madwoman - until today. It figures! Glad he stayed safe, anyway!
 
first, another test to be sure he is rising? Then do you want to monitor and be agressive? (Maybe 2.5). Play it safer - 2U. Mogs?

He is been getting some great numbers lately.
 
Okay, just tested again before sticking dinner under his nose - he's at 230, so pretty much the same as 30 minutes ago. Do you think he'll be safe with 2.5? He went down to 69 (Relion meter) the night before last on 2.75U.

He should be, but if you give 2.5, I'd be prepared to test early on and maybe at nadir. If you want to sleep , do the lower dose. As always, you hold the syringe. Go with your gut.
 
Maybe split the difference and go with 2.25? I've been really lucky that he has handled low numbers pretty well, so far, but I don't want to push my luck too much, lol.
 
As you both know, I'm always quite conservative WRT dosing. I also worry when unexpected oddities crop up - especially when the oddity is markedly lower than recent data.

With both the low green nadir yesterday and the low PMPS tonight, my feeling is that it might be safer to drop back to 2 IU or possibly lower (2 IU has dropped Squallie's BG significantly relatively recently). There's no depot to consider and you can always bump the dose back up a little tomorrow. If Squallie was mine I would definitely step back from the 2.75. With nadirs like yesterday's AM cycle the 2.75 is not leaving much safety wiggle room for a cat on Vetsulin.

How much you can monitor should guide your choice.

Oh! Just realized I tested him before lunch today! He was 141 at +5. (That'll teach me to fill out my SS in a more timely fashion!)
 
@Critter Mom Mogs, if he was yours, what dose would you give? I can stay up/wake up to monitor him, but I still don't want him to drop too low. Just as an added observation, he's been little clingier today, and doesn't seem to have the same voracious appetite he's been having.

It could be anything *sigh* from being cold (our house is old and drafty) to being afraid Candy will ambush him, to not feeling well. He's acting normally otherwise, maybe a little more active today than lately.
 
If he's not as ravenous then it's possible he has been in better numbers. Saoirse does similar. If he's a bit more active that again might point to his body using the blood glucose more efficiently.

Dose-wise if he were mine and I was sure I could stay awake to monitor maybe 1.5 - 1.75 IU. If I thought I wouldn't be able to monitor then I would most likely do a Big Chicken Shot of 1 unit. (But remember I am an insulin wimp.)

Well, I'm a big insulin wimp, too, lol. I decided to just give a skinny 1.75 and monitor him. I'd much rather he be high and safe than too low and at risk of a hypo. Thank you for the help, like I said to Sue, that 224 really threw me!

How are you and Saoirse doing tonight, Mogs?
 
I'm utterly, utterly exhausted, Lucy. Overall, Saoirse is brighter in herself but she does get cranky sometimes; usually after eating and very much so when I'm trying to clean her derrière. (Oh, the indignity of it! :oops:)

How did you get on the other day? (Sorry it has taken me so long to enquire. :oops:)

I'm so glad she is doing better, Mogs! And I'm sure you are beyond exhausted. I hope you can get a bit of rest now that Saoirse is getting better. What a terrible scare that must have been that first night! I am so sorry you had to go through all that, I know it must have been terrifying. That kind of scare can be very hard to shake, but I hope you will gradually be able to put it behind you.

My day at the hospital went very well, thank you for asking. I am trying to qualify for a study of a new procedure that has been approved in Europe, but not here, yet. After several trips to the hospital and endless tests and whatnot, I think I'm in! Fingers crossed, anyway!
