Need advice please - 1 week check up after diagnosis

Hello all. I’m hoping for some help/advice for my kitty. Sorry this is so long!
Here’s the situation: this is a male cat, 7.5 years old. He’s a big cat and I knew he was a big overweight but I didn't think it was that much. He’s an indoor cat, has been fed on Purina Indoor formula most of his life with a small amount of canned food. Until recently this was basically 1 can of Meow Mix or Fancy Feast each morning of which he would eat maybe half. The rest was the dry food. A few months ago I started switching his food from the Purina indoor formula to Iams Weight Control. I started with about 5 to 1 ratio or Purina to Iams and slowly added in more and more of the Iams. About 3-4 weeks ago he was at 75% Iams and 25% Purina Indoor Formula. No change in the canned food at that point. I realize now that switching him to Iams was probably a bad move!

About a week and a half ago I started noticing he was drinking a lot more water and within a couple days I saw quite a drastic change in his water drinking and peeing. I took him to the vet and the next day we had results back from the blood work. Everything was fine except for his sugar level was at 500+ (not sure the exact number, I think around 530 or so). He was diagnosed as diabetic and the vet started him on 1 unit of Lantus every 12 hours. She also said I should cut out all dry food and recommended I feed Fancy Feast Classic varieties, twice a day for total of about 350 calories per day. His weight was just over 26 lbs and she wants him to get to around 20 lbs. (last vet visit before this was 2.5 years ago and his weight at the time was 21 lbs. He was already a bit over so probably 18-19 might be a good weight for him eventually). I was worried about starting him on insulin at the same time as switching his food but she said due to his high levels, she thought it best to start him right away with a very small insulin dose for his body weight.

This is now a week later. He’s been doing fine, eats his canned food well. I can’t really get him to eat 2 x a day so I modified this a bit with smaller meals throughout the day. I have more questions about this but will skip over this for now. Other than the excessive water drinking before the vet visit, his behavior was always pretty normal. The excessive drinking stopped right away after starting the insulin and his behavior continues to be normal, eating well, affectionate, normal activity levels for him, etc.

My follow-up visit to the vet was today and his blood sugar was checked about 6 hours after the shot (about 10 minutes shy of the 6 hours). His blood sugar was at 136. His weight was 23.6. Vet’s directions were to continue the 1 unit of Lantus and bring him back in 2 weeks for a re-check.

So now I’m concerned about a couple things…first, I’ve read that it’s not good for cats to lose weight too quickly. One place I read said they should lose no more than ½ lb per month or risk some kind of liver disease. I actually didn’t get to talk to the vet but talked to the assistant but it didn’t appear there was any concern about the quick weight loss. I do plan to call the vet with questions but thought I would ask here because I’m not sure what to think. Also, with the lower blood level now, I'm more concerned about a low blood sugar episode (this hasn't happened as yet, just worried that it might). Since I give him the shot at 6am and leave for work around 7:30am, I am worried he could go too low when I’m not around. Same thing at night when I’m sleeping. I’ve read here about home testing and I’m planning to get a monitor on Friday and see if I can brave myself to do this over the weekend. Meanwhile, should I continue to do the 1 unit every 12 hours as directed? I know it would be best to get the blood sugar level before doing the shot but unfortunately I have to wait till payday in order to get the monitor and supplies. I’m not even sure he will let me do it so not sure how successful that will be. If I can’t get his levels, do I still continue to give him the shots? I would really appreciate some advice!

Welcome Angelica(?) and what is your kitty's name? Great job on the vet recommending you switch to low carb, wet food. Here's a fantastic website written by a vet all about feline nutrition that explains why a low carb, wet food diet is good for all cats.

This is actually what we generally recommend for our diabetics. Small, frequent meals throughout the day and night seem to help support the pancreas better, so if your cat is a grazer, it's fine to spread out the feeding. You can also add water to the wet food to help stretch it out further and keep it from getting dried up and gunky. Plus, it helps flush the kidneys and keeps them well-hydrated. ;-)

No, you don't want your cat losing weight too quickly, however, because of his diabetes, rapid weight loss is often a side-effect. Just keep an eye on him and make sure he doesn't continue losing weight too fast.

That is a potential for concern, however, because he's just been newly diagnosed and because he's on a good starting dose for his size (1u), this is a lot less likely to happen. The sooner you can get started home-testing, the better. In the meantime, check out the how to treat hypos link and make sure you stock up on some higher carb foods and have some honey or karo syrup on hand.

As to glucometers, depending on where you're located, many of us here in the US use the Walmart brand ReliOns because the test strips are so much cheaper than any other glucometers. In Canada, many people use the Bayer Contours.

Yes, I'm very glad my vet's advice seemed to go very well with what I read on the site. Actually, she printed out some info from this site and directed me here. I've been trying to take in all the info since then but of course, it's a lot to learn! So glad you guys are around to help us newbies!

I"m going to start weighing him at home to get an idea of how much he's losing every 2-3 days. If he continues to lose weight quickly, should I increase his food a little? He's basically eating about 3.5 to 4 cans of Fancy Feast pate varieties per day. I do add a little water to his food, he does seem to like that. I was a bit worried about spacing out his meals but it didn't seem likely that he would convert from 7+ years of grazing to eating big meals twice a day all of a sudden. Thanks for your input on this, that helps.

I'm also in the US (Arizona) and I do plan on getting the Wal-Mart Reli-On monitor. When I looked online, I saw there was a couple different models...is there any preference? Looks like one is "Prime" and one is "Ultima" not sure of the difference.

I read that you always want to do a treat when doing blood checks to help kitty tolerate them better...any thoughts on what to use? I couldn't find any sort of freeze dried treats at the pet store - either dog or cat section. I thought maybe some lean deli turkey? I've never given him that but he seems interested when I'm making a sandwich!

Unregulated diabetics are basically starving because their bodies aren't able to process nutrients properly. Feel free to feed him a little bit more if he'll eat it and you might also want to read up on this discussion on feeding kitty as much as they want for some more ideas.

The ReliOn Prime requires a bit more blood, but the strips are the cheapest ($9 for 50)
The ReliOn Micro/Confirm requires the least amount of blood (strips are $36 for 100)
The ReliOn Ultima...I'm not sure about this one since they don't seem to carry this in any of the Walmarts around where I live

Deli meat usually contains sugar so it's not advisable long-term, but if it's all you have at the moment, it should be fine. Here's a list of treats that many of us use here. Basically any freeze-dried pure protein will work. Make sure you check the ingredients list and it should only have the one protein ingredient and no other ingredients. I also will use plain boiled chicken breast sometimes, but my kitties really like the crunch of the freeze-dried treats, so I use the chicken as more of a protein supplement to their food than as a treat. ECID! (Every Cat Is Different.)
 
Thanks again, that was very helpful. I read thru the discussion regarding feeling cats as much as they want and that was very interesting. I think since Hershey lost almost 3 lbs the first week, I will continue to let him eat throughout the day and set out a little more food, especially during those times when he tends to lick the bowl clean. Other times he doesn't finish it so basically I just need to let him guide me and see how it goes the next few days.

Thanks for the post about the treats also. I checked Amazon and they carry some of those so I will order as soon as possible.

I will read thru all the info on home testing and hopefully try this Friday evening or Saturday.
