Need advice on treating chronic constipation

Riley went from slightly loose BMs to no BM for several days late November. The vet took an X-ray which showed 3 separate clumps in his colon. After an enema he released one of the clumps. The other two followed later at home. He was prescribed 2 ml Lactulose twice a day plus 100 ml sub-q fluids every day or 150 ml every other day. The 2 ml Lactulose didn't work quite as well as hoped. He saw a specialist vet at regular vet's request who ran tests and did an ultrasound, but never gave a diagnosis - only suggested adding 1/4 tsp Miralax. This didn't improve things. We changed vets and the new vet said to use 3 ml Lactulose twice a day until things got moving, then quit using the Lactulose because it could cause diarrhea. Adding a little more Lactulose seemed to help but I didn't quit for fear of diarrhea, just cut back to 2 ml. His BMs are usually Number 1 on the scale, but occasionally he scores at Number 2. Until last week. He hadn't gone for a couple of days and then started straining, straining, straining, and straining no matter where he was - even in my lap. Some liquid was escaping but no stool. Off to the new vet, hoping she'd take an X-ray to see what was going on in there if he needed an enema. She took him to an exam room without me and told me that when she started to examine him he produced a normal BM. She said his stool was a normal softness and I shouldn't be worried. But I am because he has produced exactly 2 Number 1 stools since Saturday. I am thinking about increasing the Lactulose to 3 ml twice a day. Any suggestions? Pumpkin - he likes it from a freshly opened can. Since he doesn't finish the can all at once I save it and offer it later and he refuses it.
 
Try freezing the pumpkin in ice cream trays. And getting out as needed.
I used to cook my own pumpkin in water, nothing else added then mash and freeze. Then used twice a day in food.
