Need advice on re-visit to vet

I have posted a few times in past 3 weeks about Milo who has been OTJ for 5 years. Around Thanksgiving a routine check got 477 reading. Around the same time he was grooming excessively and we found small 'pimple' like dots on back and tail. Took him to the vet 2 weeks ago yesterday. He gave a flea treatment and antibiotic shot. He honestly didn't see fleas (we have had a hard freeze and Milo isn't an outdoor cat just goes outside as a treat when nice out). We had been low carb fancy feast. I found even lower carb food on the list and he has been eating that for 1.5 weeks. I did post last week he got in the garbage (last Monday night and ate some turkey and noodles). I have been checking levels and he is still 170-220 level. Only symptoms I have seen are drinking a little more (he hardly ever drank water after he was OTJ) but now I just see him get smaller drinks a few times a day (I'd say he drinks maybe 1/4 cup at max daily where when he first was diagnosed it was an entire bowl). He is still grooming excessively- he doesn't have bald spots or pick his fur out. He just licks and licks and licks until he is sopping wet which is new. There hasn't been a major change in our home--nothing I can think of to cause stress.

Am I missing something that could cause the numbers that used to be 50-80 to increase and not go back down. He has not gotten in garbage or eaten anything but the wet low carb food. His activity level and temperment have remained the same. I can't see anything different in him. I am just concerned about the levels being higher for an extended time. The vet said to monitor for a couple of weeks which I have done. I have never had a reading as high as the 477.

I am wanting to take him back but don't want to go back if not necessary. If this were you would you wait (the back of my mind tells me to BUT then I think what if he has something seriously wrong). Do any of you have any experience that is similar that might lead to clues as to what is causing higher numbers. Should I be concerned about them or just wait a couple of more weeks. I haven't seen any more pimple dots but sometimes he does get 'angry' if you touch certain spots on his back but you can't feel or see anything. Almost like his skin is sensitive.

Have they done any blood tests? Has Milo been tested for hyperthyroidism?

My cat had lumps, sores and waxy gunk at the base of his tail, kind of like stud tail, and was grooming excessively when he was diagnosed with hyperthyroidism.
They were really the only symptoms he had at the time, besides occasionally drinking a little more.
I believe that could send your cat out of remission also.

It's probably not that, but just a suggestion.
