Misty (Red’s sister) Newly diagnosed

I can understand if you are confused. Red (on the profile) was Misty’s brother and this message board was absolutely fantastic when he had his diabetes and enabled us to get him into remission in just 2 weeks. He stayed diet controlled on whiskers jelly meat for I think 5 years until he passed from an acute illness, suddenly laid down and wouldn’t eat or drink.

I will try and change the profile but not sure if I will need to reregister?

So we are now living in Queensland, Au and brought Misty (blue tortie Burmese) now 18 and her friend Blue, a 3 year old blue Burmese, over here a year ago.

Misty has been taking 5mg of Prednisalone for approx 18 months alternate days.
Approx. a week ago she started flooding her litter tray and we noticed her drinking++. Vet put her on antibiotics to see if was a urine infection. Not improved after 3 days so we took her to emergency vet.

I have included the results that were out of normal range:
Blood gas analysis- ph 7.15, k+ 3.3, Gluc 37.4, Lac 4.7,
Derived: BE(B) -15.5, HCO3c 12.5
Interpretation: severe acidaemia, mild hypokalaemia, severe hyperglycaemia, moderate hyperlactaemia, decrease base excess.

CompleteBlood Count: MCHC 29.7, PLT 112, MPV 11.1
Diagnostic Biochemistry Rotor: BUN 22.4, CRE 247, Glu 36.8, Na+ 139, TP 92, T4 5
Interpretation: hypochromia, thrombocytopenia, moderate azotaemia, severe hyperglycaemia, mild hyponatraemia, elevated total protein.

We brough Misty home and changed her immediately to low carb food. Next day...public holiday! Next day I called All local vets trying to find one that would work with us to test our own blood glucose and that used Glargine. I tested her bl glucose and it was 23.8. This vet recommended inpatient care, 48 hours of fluids as severely dehydrated, treatment for constipation, continuation of low carb diet (Royal Canin wet diabetic), start 1 unit of Glargine, she weighs 4kg, stop steroids.

After 48 Hours (yesterday) they felt she had progressed well and rerun some blood tests.
SDMA 14 normal, CREA 247 high, Urea 11.9 normal, BUN/CREA 12 normal
Blood glucose 3.8

We were told not to give insulin if under 15. Tonight tested. BG 24. Gave 1 unit, although used insulin pen for 1st time!! 2+ BG test 19.

I will not be able to put as much time and sleep deprivation into trying to get Misty into remission but will certainly have a go. Are the chances less as she is older and obviously has a few other health things going on.

I would set up a spreadsheet for Misty so we can see what her BGs are doing.
Are you giving her the Lantus 1 unit twice a day?

Did the vet mention ketones to you at all?
I think it would be a good idea to test for ketones. You need to buy a bottle of Ketostix from a pharmacy...about $12 for a bottle of 100 test strips. Then you collect a urine sample from Misty and dip the test strip into the urine and read the result against the colours on the side of the bottle. Anything above a trace needs vet attention.

Are you able to upload the actual path results here? We have different values our here to the US and it would be much easier if everyone could see the normal values that are put beside the actual result.
The SMDA of 14 is the upper limit of normal.

I dont think she has any less of a chance of remission because of her age. It is how damaged her pancreas is and if it can recover. Only time will tell.
I am going to tag @Marje and Gracie to have a look at the path results but could you try and get them uploaded so that she can see what are the normal values for Australia.....Marj lives in the US.
 
You might be better off starting a new spreadsheet from the beginning. Our instructions have a link to a template that includes a tab for Lab values. You do have to enter the reference ranges for the lab (Idexx, Antech, etc.) by adding a column. And then add more columns for each date where you have blood work or other lab values to enter.

Here are the links with the instructions for copying our template and saving it for your own cat. Instructions included for setup on a pc, tablet or smartphone.
It's a newer improved version of the spreadsheet (SS) than you used before.
