losing weight but on regular insulin shots

Recently our Kitty has been losing weight, even with very regimented insulin shots and low carb/high protein wet food from the vet. Her demeanor and physical behavior are still all normal and healthy. Approx. a month ago I noticed she is becoming increasingly hungry but becoming thinner. I would like any info that anyone has to offer, if you have experienced this or have some insight as to the cause. She is on 2.5 units 2x a day of Lantus. Thank you.

Can you let us know whether you're home testing blood glucose levels? If yes, it would help if you could post some test results. Also, can you let us know a little bit more about: when your kitty was diagnosed; her body weight and whether she is under/overweight or at ideal weight; and also how long your little one has been on the 2.5 IU BID (twice a day) dose of Lantus.

What are Kitty's BGs through the day?
How much are yo feeding? have you tried feeding more?
When last time for vet visit/boodwork? Maybe other problems like thyroid or kidneys.
 
Hmm, I'm going thru this same thing with Goof, test results show nothing. I've
just adopted the theory that if he's hungry, I'm going to feed him, but not up
to 2 hours prior to testing time. He actually looks like he's beginning to fill out
a little bit. I'm thinking it's because wet food is much more water than actual
nourishing food, that they need much more to maintain their body weight when
wet food is all they get. I hope that statement doesn't cause a sh** storm, but
think about it, if a human drank more water than he/she consumed nourishing
food, he/she would probably be constantly hungry and consistently losing
weight, right? Canned cat food is about 75% (on average) water.

When I feed Goof between his normal feedings, it's plain tuna or chopped
cooked chicken, that way I know he's getting something besides more water.
 
Weight loss is very common with hyperthyroid. You could check with your vet and see if the tests were ever done. They are normally specialized tests and would not necessarily be included as part of a complete bloodwork package.

Yup, that test was done and was within normal range. She did say that even tho
the Cushing's test came back negative, it could be that he's just in the early stages
so it may not show up yet. If this new plan of feeding him whenever he's hungry
doesn't pan out, then we'll test him again a little further down the road.

I do test at home with a Bayer EZ Contour meter twice a day, her counts range from 15-24, and the vet had said that anything above a 7 requires an insulin shot. I notice alot of people posting numbers in the 100's so I am assuming that this meter I have converts it differently. She has been on 2.5 units for about 4 months, started at 2 units in July of 2015 when she was diagnosed. She was not a heavy cat ever, and I was shocked when this all happened to her. She is a tall and long build of a cat. I have not had her weighed since January, I am noticing when I look at her that her weight is definately down. The last time I had her weighed I believe she was at just around 4.8 or 5 lbs. I feed her just over half a can of diabetic Hills DM cat food twice a day. Hope this is enough info...

My kitty had a raft of Cushing's tests done and most of them came back as borderline. However with 2 U/S tests and the clinical symptoms even my doubting vet is calling Tuxie as early Cushing's Unfortunately the reference points for Cushing's tests in cats is not well established, due to very little research and many labs use the ranges for dogs as their guideline.

Hi Patricia,

The reason you're seeing people talking about numbers in the 100's is because the United States uses a different measurement of blood glucose than we do internationally - they use mg/dL, while we use mmol/L. That is why you're seeing a significant difference in your numbers compared to much of what's talked about on this site. If you're interested in comparing, you can multiply your blood glucose results by 18 to get your result in mg/dL.

I want to mention that I don't have as much experience as others here, but one thing that jumps out at me is the blood glucose range you posted - those numbers look high, especially that 24. Numbers that high would mean to me that, while Kitty is getting regular insulin, her blood sugar is not well regulated on her current dose of Lantus, which could account for her weight loss. Insulin allows the body to use glucose for energy, so when we see high blood sugar numbers, it means the body isn't using up that glucose - this can lead to weight loss, despite being fed what should be enough food.

Do you ever do any additional blood sugar tests on Kitty? Lantus dosing is mostly based on how low it takes a cat's blood sugar during a 12 hour period (lowest point = nadir). Nadir is generally around the mid-point in the cycle (5-7 hours after giving insulin), but can be earlier or later that that as every cat is different. Those mid-cycle tests will help you figure out how the insulin is working and allow others here to help you find the optimal dose for your kitty. I don't give dosing advice at all, so I'd rather leave that to more experienced people.

It may help you to post over on the Lantus board - there are many very knowledgable people over there who will be able to give you a hand. http://www.felinediabetes.com/FDMB/forums/lantus-glargine-levemir-detemir.9/ If you do make a post over there, it would help to include a link back to this post so that people can get an idea of what's going on.

In the meantime, hopefully bumping this up will get some additional eyes on things.
 
Also, what wet food from the vet are you feeding? If you're feeding Hills M/D, that could be part of your problem, because it's not actually low enough in carbs for diabetics.
