Little Ra is starting Lantus! Questions...

Hello all!

My vet was gracious enough to switch over to another insulin (Lantus) from Vetsilin. I am a bit unsure as to what dose I should start my cat at. According to the TR protocol, I should start my 5.4kg kitty at .25 units per kg correct? So 1.25 units? I plan on switching his food to low carb wet food in the next few days as well. Do you think it would be safe to start him on Lantus and change his food at the same time at this dose with periodic monitoring? Or would it be better to wait?

Sorry if this is rambling. I know just enough to be dangerous!
 
Welcome! I think the protocol is different when switching insulins? I switched Cameron from Vetsulin to Lantus last month (second tab on his spreadsheet shows where we switched) and he had been on 1u of Vetsulin, but the folks here suggested 0.75u to start the Lantus. I've been really happy with the results, although it's been quite a learning curve and a little bit scary at times.!

When switching from one insulin to another, we take the current dose into consideration. Since you're already on 2U of Vetsulin, you might want to start at 1.5U

You will need to try to get at least 1 mid-cycle test (between +5 and +7) on the AM cycle and at least a "before bed" test on the PM cycle, along with your Pre-shot tests, especially if you're going to be reducing the carbs in the diet

What a cutie pie he is! How did you discover Little Ra was diabetic?
Any other health issues?
Transitioning to a long acting depot type insulin requires a period of closer attention. For safety sake get a BG test before each shot and test each day for the presence of ketones.

Changing to exclusively low carb wet is a good move. It can be a huge game changer. That being the case I think it would be wise to only change one thing at a time.
I'm thinking start the Lantus first and once you get a feel for how it is working we can help you with a strategy for the change to all wet LC and the TR protocol.

Thank you for pointing this out! I saw something like this in the "Go slow" method and it really had me wondering.

Ra thinks he is a big cutie pie too! Thanks! I will start the insulin, then move toward low carbs for sure. I took the Little man to the vet because he looked sluggish and was sneezing. He has no other health issues as far as we know.

Best advise EVER! It applies to almost everything you do in the dance :)

Admittingly, it is very hard to do :banghead:

Just want to say welcome! Keep posting and asking questions as that is how we all learned. We are here to help. If you would like, many of us post daily. We start a new thread for each day and in the title put Ra's name, date, the AMPS = xxx. We will help to guide you along the way. Also, could you put your location in your signature? Welcome! We also switched from Vetsulin to Lantus. We were at 2u of Vetsulin and we started at 1u Lantus on the vet's advice. In hindsight, we would've been better at 1.5u.

Little Ra is a cutie. He reminds Bobbie of Grandpa, which I can see, but he really reminds me of my Regal. Here's Grandpa with Regal.
