limit on dose eventual dose

hi there,
i have a 15 yr old diabetic kitty (meepers); he is 1 yr into his diabetic marathon; we were well controlled on 3 units lantus (twice a day); until about 1 month ago; he had an episode of anal sacculitis that became inflamed and ruptured, now his numbers keep rising; the vet has had me raise his dose to 4 units (twice a day), 2 weeks ago; and we are still not seeing any numbers come down. Is there a cap at which you should not continue to raise the lantus dose? i thought 4 units were getting to a level that might be too high, my previous vet said he did not like to go above 3. with the pandemic i have not even been able to talk to this new vet in person, only through the vet tech who comes out the car to pic meepers up. any thoughts would be appreciated - thank you :)
also, just a note during this time frame we have changed foods due to another cat (bones) IBS issues - he used to eat: Hill's Prescription Diet c/d Multicare Urinary Care with Chicken Canned Cat Food; but is now eating: Instinct Limited Ingredient Diet Grain-Free Pate Real Rabbit Recipe;
I'm wondering if maybe he's not getting enough carbs from the new food, as backwards as that might sound.

The best dose for your cat is the dose that controls the numbers. If you look at member's spreadsheets, you'll see a range of doses. Some cats are at high doses, some at lower doses. It all depends on what your cat needs.

The first thing that comes to mind, though, is related to the anal sac issue. Is there still a chance there's an infection or inflammation present? Any infection/inflammation will cause blood glucose (BG) numbers to rise. The other concern I would have is whether you're getting any spot checks during the cycle. Lantus dosing is based on the lowest number in the cycle (i.e., the nadir) and not on pre-shot numbers. For example, the drop to 174 on 6/4 may have caused the subsequent spike -- you don't know if numbers went lower than 174 and there's a rise in numbers by the PM cycle.

You may want to read the New to the Group sticky. We all use spreadsheets to track our cat's progress and having a spreadsheet would make it much easier for us to provide you with feedback. The sticky notes at the top of the board are a bit overwhelming but will also give you a great deal of information about how Lantus works.
 
he did get another shot of convenia on 6/1/20 just in case, and i will be taking him back into the vet on 6/15/20; i am thinking its related to the sac issue as well, he seemed fine until that happened. i'm not home during the day to check him like that; i squeak in the door just in time to get him his shot. i can put what i have above into the spreadsheet format if that's easier, thank you.
