The story so far -
Liam started Senvelgo in March. He gets his vet care at a pretty large practice in Washington DC, so sometimes we don’t get our regular vet. We started with a relatively new vet at the practice that was pretty dismissive of ketone testing at home and seemingly unaware about Dear Vet letters from the FDA and the equivalent warnings from the EMA about concerns with DKA.
The first package of Senvelgo I got was missing the insert so internet searches got me to a wealth of information.
I could see why this new vet thought this was a good plan. Liam appeared to be a good candidate for senvelgo with low ketones .4, but he has a history of pancreatitis - and she didn’t have the dots to connect.
We had another go with Senvelgo, the last (May-July) under our regular vet after more lab work re: pancreatitis and Liam seemed like we were ok. Then my petsitter ended up bringing him in for vomiting while I was out of town in July. He was ok, Cerenia injection, fluids and stopping the Senvelgo AGAIN put him right.
Tried to restart mid July, more vomiting. Our vet agreed - insulin seemed like the best plan. We had an appt on the books for 8/1 for Liam and his littermate for their annuals, so I figured I’d be leaving with a script for generic lantus from Costco.
I started getting Liam used to AMPS and PMPS testing and a new food routine as soon as the Senvelgo cleared his system - and his numbers looked…. interesting. Like if I gave him insulin I’d be risking hypo.
On approval from our vet we stopped the royal canin urinary SO pate wet food this weekend (Liam had a blockage in 2022) and made the wet food switch to lower carb wet food. Liam is also eating Wysong Epigen 90 for day time and late night grazing. His autofeeder locks him out at fasting times (3 to 6 AM, 3 to 6 PM).

Next Saturday I’m getting more mid cycles during the day for 12 hours to see what his BG does when he is eating. Jury is still out on insulin and how much. Clearly he is making insulin but hopefully with his new diet change he can use it more effectively.
TLDR - remission or not, Jury is still out!
