Levemir-2nd remission?

Hello Everyone!

My Pudge fell out of remission in May of this year, right after a terrible bout of diarrhea during which he stopped eating, drinking & peeing. I syringed food & water but he wasn't getting enough nutrients, so I gave him some dry kitten food. He ate that, but after a couple of days his BG was about 250. Put back on wet FF chicken or turkey, his BG went down to below 100 but stayed at @ 150 an hour or so after eating. He's very carb sensitive, as well, as a grazer. He's 13y/o, DX Dec. 2011, 1st remission mid-2012 on ProZinc. The second time around, I had Levemir flexpens available so that's what I used (I have a 2nd DM kitty, Little One, who's OTJ off Levemir). He has not had any insulin since a drop on May 30 because at that time his (AMPS or PMPS taken with no food during prior 2 hours) BGs were low: 86, 76, 89 & similar, even a scary dip into 40s. No two ways about it, the shed scares me. He's still refusing to eat Before Grain chicken or turkey, hates his ears poked but does not mind injections.

My questions: (1) I plan to test only once a day for the next 2-3 weeks, if his BGs start going up, at what number should I consider restarting insulin? Over 130? Or? I plan to restart with a drop of insulin or would it be better, due to the shed, to restart with 0.5 unit? (2) During his 1st remission, his BGs were in the 40s, similar to his 2 littermates.' My triplets were rescued as feral kittens. Is it common for 2nd remission to result in higher BGs? Or did I simply cut off the insulin too soon?

Although I realize it's best to ask questions when working on an issue, my computer is a piece of junk so I can't always log on. Guys, don't buy an A--r! Any advice, suggestions, hints will be gratefully accepted. Thank you for your time.

Not sure i understand where his BG is now,, are you saying 100-150 while not on insulin? Maybe a food change will help tweak him down a bit. Something lower carb again, and frequent mini meals during the day.

What dose of levemir did you give that resulted in the recent scary drop?

I think if you see 3-5 days with numbers consistently over 150 you should start to reconsider insulin (and/or a food change to lower carb) but it would help us to watch his numbers too.. Can you set up a spreadsheet? http://www.felinediabetes.com/FDMB/viewtopic.php?f=6&t=18207

Thank you, Wendy. Pudge's current BGs, over the past 20 days with no insulin, have been between 75 and 112 (mostly 2 digits) taken at different times during the day & night. Normal range but taken only once a day while he free feeds. Good reminder to lower the carbs, I'm adding a bit of Before Grain to FF classics. He's a FF fancier & stubborn. Plus, his big blue eyes get to me big time! I'll try for 0 carbs.

The scary drop was on 0.5 unit of Levemir which I immediately dropped to 1 drop. Thanks! I'll remember to start with a drop only & make sure that some 5 days of higher numbers have gone by before reconsidering insulin. I have 2 great big notebooks of notes on Pudge & Little One which I have never entered into their ss while promising myself to do so in the future. My fingers are a problem for me - I'm 70 y/o now & my typing plus computer suck...big time. Can't participate as much as I'd like since I'm worried about forgetting a suggestion & not being able to reconnect. I'm dreaming of an Apple Air computer in my future. Thanks, again!

If he is 75-115 he does not need insulin or even a food change but its a good idea to keep monitoring, maybe once a week, to see how he does. If he does creep up and spends most of his day over 130 then come here and we can help you out at that point and advise.

Also keep an eye on him for any signs of infection - dental, ear, urinary tract.. that can spike blood glucose too and cause him to fall out of remission.

Paws crossed he stays in remission though!

Wendy, thanks! Pudge is still doing fine. Will keep an eye on his BG levels & possible infections. It appears to me that speedy response with insulin seemed to help him into his 2nd remission, although I'm just as sure that won't be true in every case. That darn ECID! I'm thrilled that my fridge is graced by fresh Levemir flexpens with exp. date of Sept. 2014!!!
