I have a quick question regarding duration of a Lantus dose. The information I'm reading says it can last 12 hours which is wonderful news. What scares me is if I am not able to make it home from work within the 12 hour window. Does anyone know if 15 or 20 minutes would cause damage? I need to figure out my work schedule and adjust, especially if I have to accommodate shots 3 times a day.

If you choose to go with 3 shots a day, you won't be able to use Lantus or Levemir because of the shed. You will have issues watching for nadir and could run into issues with the carryover and crossing nadirs from 3 shots a day.

If you are 15 or 30minutes late with shots when on Lantus or Levemir, it's fine. I am sure that many others have the odd days when their shots are off the 12/12, due to just ordinary life events.
People aim for the 12/12 but you certainly have room for wiggle.
 
while there are sites out there that do promote shooting lantus 3 times a day, in my own opinion, it's quite risky. i want to say but i'd have to re-read to make sure, even those sites, if one shoots lantus 3 times a day, say that you actually have to test even more so you get a better feel for the insulin reaction and to make sure kitty is safe because of issues like Gayle mentioned. if you aren't home most of the day or can't run home for checks thru the day, i'd say forget about trying a TID schedule with lantus.

with that in mind, i use lantus and thru the week i never shoot on a 12 hour schedule. work doesn't allow for it. while it may not be ideal, my cat and now my foster cat, seem to do pretty well with it and i've never had any complications due to it.

thru the week i shoot my evening shots as close as 12.5 hours one day to as much as 13 or 14 hours some days if need be. of course that makes the next shot alot less than 12 hours apart but it can work. you have to learn things like when it's safer to skip a shot because the next one is coming too close or perhaps reducing the dose a little to be able to shoot again the next morning etc.....the data you can collect in the evenings, or on the weekends and holidays, will help you with that. i will say that if your big goal is remission as quick as possible, (which personally i don't think should be the goal) this kind of schedule can and most likely will slow that down some.
 
Lantus is, of course best if you can get as close to 12/12 as possible. When we are moving a shot time we go as much as 30 minutes without worrying too much. If you need to be off by much more than 30 minutes, it is usually best to move your next shot too if possible so that you are never shooting much outside of that +11.5-12.5 window. It can act like a dose increase or dose decrease to be farther out than that. Some people will adjust their dose slightly up or down to account for being off on the time, I prefer to adjust the time but do have a flexible schedule.

As others have said, Lantus is not an insulin to be used if attempting to dose 3X per day. It is a long lasting insulin with a shed and 3X per day dosing is usually only done with shorter acting insulins without sheds so you don't have to worry about a long overlap. Putting more insulin in at 8 hours when using a 12 hour insulin can be dangerous, you can't get it back out.

I have to agree with the others. While my use of Lantus was short, there were times during that month when I had to adjust the dosing time because I had to be in to work at 5 or 6 in the morning. cedric took it in stride as he was used to getting fed at 3am for 2 days a week for over a year, so it was just a matter (for me) of adjusting the other days slightly so it wasn't an absolute 3 hour difference (used to be 6am 6pm).
 
You do have some leeway in dosing times with Lantus, especially if you are hometesting. My schedule is erratic also and often Ricky gets his shot a couple hours late. As long as there are at least 10 hours between doses and when you test him, his BG levels are above your don't shoot number (usually 200 for newbies), you should be fine.
 
HI Kim, I am a current Lantus doser (I was going to say shooter but it sounded bad LOL) and do a 12 hour shot schedule. I am sometimes late on shots but mostly get it down within an hour of the 12 hour mark. Sitka hasn't had any adverse effects and since I go by her blood glucose levels at test time she hasn't had a hypo incident from it. Her sugar levels are a little higher if I'm late but thats been about it. I wouldn't shoot Lantus three times a day though since its really not designed for that type of use. Jan
