Kitty has been OTJ for 6 months - now having issues

We are not sure what is going on with our Evie. She was diagnosed diabetic 6 months ago and went into remission after 1 week. Her appetite has been great (she is always hungry) and Blood Glucose (BG) readings have been pefect until a few days ago.

She has long hair and hairballs have been a frequent problem - about one a week. On Wednesday evening after dinner she threw up a hairball with lots of food and the next morning she didn't eat all of her breakfast (very unusual) then threw up most of it (no hairball). I tested her BG and it was 200 so we went to the vet that afternoon. They said she was slightly dehydrated so gave her fluids and a shot of Cerenia. They did urine and blood tests. No ketones in the urine.

Yesterday her BG was 207 before breakfast. She has been eating better and not vomiting but she is still not up to par. Last night she was picking at her dinner so I gave her a Mirtazapine. She finished her dinner.

This morning before breakfast her BG was 190. She ate most of her breakfast today. Expecting to get the results of blood work and urine tests today.
 
How are her teeth? I know infection can cause the bg to raise, and if shes not eating as much it could also be due to painful teeth/gums. I know she's younger, but just an idea to throw out there.
 
That did occur to me. As it happens she already has a dental consult scheduled for next month (earliest I could get her in with the dental vet where her internist is).
 
That might be your issue, for the time being you may want to try mini meals. Mini meals are the only thing that seems to keep Hidey in remission and when his teeth were bad thats when I started the mini meals. His numbers just kept rising and I didnt want to give him insulin since he was so close to his dental and I assumed that was the culprit, so someone on here recommended the mini meals and I just stuck with it. (come to find out Hidey needed his upper canines removed)

as long as all blood work comes back good.
 
how do you do the mini meals? right now we feed at 6 am, 10 am 2 pm and 6 pm but the 6 am and 6 pm are bigger meals.
 
LOL yup, your basically doing mini meals already, except during the night. I just brought my cats to 4 meals from 5 a day. I feed at 7 am, 1 pm, 7 pm, and 1 am. Hideys numbers are always highest in the a.m. so I had to get an auto feeder to make sure he was getting a feeding mid-night. Hmmmmmm.....what else can you do to lower bg till the dental.......Im thinking lol..........maybe try to increase play time? or even split the meals up a bit so you can add an extra feeding right before you go to bed at night? I tried looking at your spred sheet but theres no recent info to see where the numbers are jumping.
 
Yeah her numbers have been so stable that I have only been testing once a week for quite a while now and haven't been updating the chart. I do keep my own records though so I could update it.

I could alter the mini meals so they are all closer to the same size. Right now I give her 3/8 can of prescription food at 6 am and 6 pm, then 1/8 can of Wellness core at 10 am and 1/8 can Friskies Mariner's Catch at 2 pm. I also read that higher fat foods can cause pancreas issues so I may start giving Tiki Cat for the in-between meals but I think I will wait for the test results before making that change - they did the test for pancreatitis.

She has been doing so great - it's really disappointing. I really don't want to put her back on insulin unless we absolutely have to.
 
We got the test results and it looks like pancreatitis. Her fPL is 50 and normal is 4.

She has been hospitalized since Friday evening. They are trying to stabelize her with fluids and anti-nausea meds. Her BG is still around 180 - 200 most likely due to the pancras being so inflamed. They have given her .5 cc of insulin to get it down and give her pancreas a break. She will be having an ultrasound today.
