Pretty embarrassed to post this. I have done a bad job I know. Because he gets so grumpy about testing his sugars I have just let him ride with 3 units. I went to 3.25 once and it was too much.
Because I had not been doing good testing I had them do the fructosamine. The numbers tell my shame

The notations on the left are L and H for higher or lower than last B.W. done in Oct 2021
 
Yes, you are right! Going to do that now and thanks Diane
Labs updated and the vet called and she feels like things look good but says to try going up to 3.25 and monitor.
Joe weighs 12 lbs. At last BW in Oct, he weighed 9.9

That's ok you are now going to test him everyday , don't want you to feel bad :cat:
You can get some tests in during each 12 hour cycle correct day and night

I can test 1-2 times during the day. Maybe at +6 and then again at AMPS and PMPS. One in the evening before bed
 
I would reduce to 2.75U - the 118 you got yesterday was a clue that he went lower earlier in the cycle (as with all the other late-cycle low blues you've been getting), and for safety's sake I am going to assume it was lower than 90.

Then, after a week of consistent testing we can see if we need to adjust

NO! Seriously?? OMG I have raised when I should have lowered. What about the fructosamine being so high Frost?
 
The bloodwork show some kidney degradation as evidenced by High creatinine and lesser by HIgh BUN (since Bund is raised by high protein diet), anda urine specific gravity of 1.022 and and SDMA. Potassium and Phosphorus are OK though. Total protein is at the upper limit and tends to indicate some dehydration. That can be due to increased urination caused by high glucose and some kidney degradation.

Oh I see, I didn't realize the gaps were there. Can you put a blank row in when you skip testing days? If you look at my SS you'll see how I do it since I only test weekly ish.

In that case I can't really advise on dose based on what you have.

Remember the fructosamine is an average of BG over the last two weeks. So on average he might be high, but it does not tell us how low a dose is taking him - and that's the important piece of the puzzle.

Well, just get back into testing and I won't have to shame you. ;)
I really only know what was already said, and not even that much! Sounds like Larry really knows his bloodwork stuff, and Melissa is a wizard of everything, so they gave you good opinions.
Like I said before, I think the Fructosamine is going to be high as long as he has numbers outside the healthy range, even if he does go low sometimes, as it is a measure of the average blood sugar over the previous 2 weeks.
I would focus on getting enough tests to keep Joe safe and he will quickly show you if he needs a decrease.
Don't let him get his way, he can be grumpy, but he's getting his tests!
How do you feel about his weight gain? Too much, or just right?

Hi Smorg!
I feel the weight gain is wonderful. I have been trying to get the weight back on him for a long time. He went down to like 9.2 lbs. He is a big guy, has huge feet, and is very tall. His hip bones and his spine were sticking out. At his healthiest, he weighed 13-14 pounds. So the weight gain is GOOD.
I think I read somewhere on here that an unregulated cat would/could not gain weight

Oh good, that’s great. Same boat here, Templeton lost 30% of his weight, it’s scary at a certain point. I’d prefer a little extra as a safety buffer.
Yeah, their bodies cannot process the calories properly and turn it into body mass without the insulin excreted from the pancreas is how I understand it. Once they get help from the drug, they can easily gain. Until then, it’s just a fast drop in weight and a kitty that is endlessly hungry.

Thanks Larry.
His creatine went up .5, I do not know if that is a lot or not.
His phosphorus level went down .7. It has continued to go down in the last year
