? Jean luc +11 76 AMPS Hi 1U

Here is the link to your other post here: http://www.felinediabetes.com/FDMB/threads/jean-luc-revisits-juice-12-4-amp-243-2-5u.207473/

Not sure why your vet started you back on 2.5 units. It’s higher than we would normally start here. Given that you saw a number in the 40’s before the depot was established means that dose is too much. I would reduce. How much does Jean Luc weigh? How high a dose did he get to the first time on insulin?

Yes you can shoot late, but your shot tomorrow will be 12 hours after that. We determine dose based on nadir or low points, not preshot. Are you proposing 1 unit as your new dose?

He is ~8 pounds. I’m not really sure what the dose should be. Based on the plummeting this week, 2.5 just seems to create glucose drama.

1 unit would be a starting dose for a cat his size, on a low carb wet food or raw diet, if following the Tight Regulation protocol and newly diagnosed. But Jean Luc is not newly diagnosed. We do try to be a bit more aggressive in cats returning from the Falls. Did you follow Tight Regulation last time? Is it what you'd like to follow this time? The other dosing method used here now is Start Low Go Slow, which would have a lower starting dose of 0.5 units if on all low carb food.

Last time he started with Prozinc and changed to Lantus after Prozinc wasn’t working at all. Unfortunately I cannot locate his older spreadsheets. He was on insulin for total of 6 months before remission. I think I will give him .5 units tonight. Monitor and restart at 1 u in the am. He definitely seems to be feeling better even with the fluctuations. DH will be able to help more this go road...Don’t want extra strsss and any of us with spikes and excessive dips, just trying for regulation. Thoughts?

What stage kidney disease is he? KD is very high carb food. The KD flavours seem to be in the high 20% carb.

He was eating fancy feast turkey for a long time after his original diagnosis and remission. It was throwing some odd values on his blood work. Can't remember which ones at this point cause it was a couple of years ago. He was then switched to KD. Which hasn't been causing issues. His kidney function tested fine on his most recent visit. I moved head with give 1U this am at regular time. His BG was in the high level which wasn't a surprise.

Good luck with the new dose. I hope it levels things out. That's quite the bounce!

Fancy Feast can be high in phosphorus. For kidneys a low phosphorus food is recommended. There are still several off the shelf low carb and low phosphorus options out there, if he'll eat them. Giving kidney food to early stage kidney disease, which he may or may not have is not necessary. There is a sortable food chart you can use, and search for foods under 10% carbs and low 200's or less on phosphorus. Lowering carbs might be all you need to get him back into regulation or even remission.
