Introducing Mochi (& me)

Hello all,

I'm currently fostering (with the intent to adopt) a diabetic cat. His name is Mochi, he is 6 years old, and as far as I have been able to gather he was diagnosed sometime earlier this year.

His previous foster mom had him on a diet of Friskies Prime Filets. He was getting two cans a day, but since he is a bit overweight I've cut him down to 1 can a day plus 2 ounces of FF Purely, which as far as I can tell has 0 carbs. I'm afraid to make any other changes to his diet until I talk to his vet (we have an appointment tomorrow evening for a blood test).

He's on Basaglar/glargine(?) which is U-100 insulin, but the syringes they gave me are U-40. He gets 2 units twice a day. They also gave me another box of syringes, but they are U-100's. I'm not sure if the rescue knows there's a difference and I'm worried about what to do when the others run out. I will ask the vet about it when I'm there tomorrow though.

We had a scare yesterday when he went HYPO (due to my own stupidity), but - thanks to the information on this site - we got through it and he is doing much better today. I recently bought a glucometer and hadn't had much luck getting blood or, when I did, getting a good reading. But desperation is an excellent teacher apparently. His poor ears are all bruised now though.

Are you using the U40 syringes to dose the glarine? This could be very dangerous unless you're aware that as a U100 insulin, glargine is 2.5 times more concentrated than a U40 insulin. If you fill a U40 syringe to the 2 unit mark (second whole unit line) with glargine you are, in fact, giving 2.0 x 2.5 = 5.0 units of glargine. This could be dangerously high. Please confirm you understand what I'm saying.

Welcome, Kimberly & Mochi! It is very kind of you to foster a diabetic cat. Have you seen the cat food database yet? Those Friskies Prime Filets are 14 and 15% carbs. Your choice of FF Purely is much better. Good thing you realized those syringes were different, maybe you need to educate the rescue. There is a conversion chart. I see that Kris just posted about the syringes - she will help you out. She is very helpful to me. Also, you can post any questions like that on the Main Health Forum where they will get more attention since this is an introduction forum.

Is it the spreadsheet link you are having trouble with? Maybe post about your issues in the Tech Support Forum.

Yes, I understand the difference. That is what the rescue was giving him when I took him in. 2 units using the u-40 syringes. I have no idea if that is what he was originally prescribed or not. I will talk to the vet about it when we go tomorrow, because I am obviously lacking some vital info.

He seemed to be doing well up until yesterday morning and that was my mistake for trying a different food (lower carbs) without testing him first. Lesson more than learned.

Yes, I have seen the charts and I made myself a list to take with when I go shopping. I may have to educate them, yes. My daughter says that Mochi is lucky we decided to adopt him so he can get the special attention he needs. Not that the rescue ladies aren't wonderful, they are just stretched thin I think.

Had a peek at your SS and I think the 2 u dose is too high. Even if the BGs were partly very low because you fed a very low carb food, the dose needs to go down. He's higher today because he's in rebound from those lime green numbers. We usually recommend changing in 0.25 u increments. The depot associated with the 2 u dose will still have an effect for a couple of cycles ahead so keep that in mind. I'd try 1.75 u tonight. The Lantus forum is very large and has lots of expertise to draw on.
