Insulin Resistance

How common is Insulin Resistance in older DM cats? My 12 year old male Abyssinian cat has started with 2 units twice a day of Lantus going up a unit ever 10 days, and we are now at 6 units and no change at all. Will get some blood work today, but what are likely causes? Using RX wet cat food during this time.
 
That’s a lot of increase very fast and you could very well have missed the ideal dose by increasing by full units each time. That’s why we recommend increasing or decreasing by .25 increments. Also typically you’d have started Lantus at 1 unit not 2. I started thinking that about my cat too as we went all the way up to 9 units a day before I found this forum and started home testing. She was down to 5 until recently when she had pneumonia and her numbers are a bit all over the map still because of that. it’s just an example of how your cat could need less not more, but the only way to really know is to monitor at home with testing :cat:

Are you home testing? Do you have blood glucose data you can share?

Yes. I strongly recommend home testing. It is very important.

My cat was diagnosed one month ago. I slowly transitioned his diet from dry/canned food mixture to all wet canned food. And his insulin dose has decreased from 2u twice a day to 0.1-0.15u twice a day now. Had I not done the home testing, there would have been no way for me to know how much insulin he actually needs.
 
There was no sign of hypoglycemia and it seemed like no effect called for more. It was 10 days of twice a day doses before moving up. I just got a home testing kit. The tell tale sign is drinking and peeing which have been large and constant irrespective of the dose level. Did an 8 hour glucose curve at 3 units twice a day and was about 350 - 400.
 
I’m not sure because Minnie also had all the behavior issues still until we got to 9 a day and decreasing gradually as I said to 5 and she’s doing even better. I’m convinced she must have had a few overnight hypo incidents at that high dose that I just didn’t witness... anyway, let’s see what others more experienced have to say. It sounds weird. Have you checked the expiration date of the insulin? Is it still good for use? Was the insulin bottle well sealed when you got it?
